Holidays--All full time employees receive ten paid holidays per year. (Personnel Policy 403.3.3)

Vacation--Paid vacation is earned at the following rates:

Part time employees--Earn six hours for every 160 hours worked.

Full time employees - Part time employees-

8 hrs./month 1st and 2nd years (12 days=96 hrs.) 6 hrs/160 hrs. worked 1st and 2nd years

10 hrs./month 3rd through 5th years (15 days=120 hrs.) 8 hrs/160 hrs. worked 3rd through 5th years

12 hrs./month 6th year on (18 days=144 hrs.) 10 hrs/160 hrs. worked beyond 5 years

There is an accrual limit of what can be earned in 12 months. Unused vacation time is reimbursed at the time of separation for employees who have worked at least one continuous year.

Sick Leave--All full time employees earn paid sick leave at the rate of 8 hours per month. There is an

accrual limit of 40 working days (320 hours).

Group Health Insurance--Group health insurance coverage through Wellmark Blue Cross Blue Shield is available to all full time employees and their dependents. Coverage is effective the first day of the month following a 60 day waiting period. Enrollment must be completed within 30 days of date of employment.

An employee who acquires dependents through marriage, birth, adoption, etc. must enroll them within 30 days of the event. Coverage is effective on the date of marriage, birth, or adoption.

Flexible Spending Plan Employees eligible for the group health insurance are also eligible for the flexible spending plan. Effective date is first of the month following a 60 day waiting period. Wellmark Blue Cross Blue Shield is the Administrator of this plan. This allows employees to pay a wide variety of expenses with pre-tax dollars. The expenses fall into three categories: 1) Insurance premiums, 2) Out-of-pocket medical expenses, and 3) Dependent day care expenses. Group insurance premiums will be automatically pre-taxed unless a waiver is signed.

Life Insurance/ADD Coverage—EMC National Life Company is the insurance provider for Life/Accidental Death & Disability coverage for full time employees. This policy includes $20,000 Life/$40,000 ADD coverage. The Agency pays 100% of the monthly premium for the employee’s coverage. Coverage is available the first day of the month following the employee’s start date. Dependent life coverage is available at the employee’s expense. Dependent/Spouse coverage is limited to $2,000/Spouse and $1,000/Child.

Dependent Life Premium Rate: $ .24 per paycheck

Voluntary Group Term Life Insurance and Whole Life Insurance Coverage

Full time employees are eligible for additional life insurance for themselves, spouse and children. Rates vary according to age. See Human Resources Manager for details.

Retirement Annuity--All employees are eligible, on the first day of the month following their date of hire to defer wages into the LIFEQUEST annuity retirement benefit plan. The employee may begin contributions of 1-15% of his/her salary to the plan at the beginning of any quarter (January 1, April 1, July 1, October 1). Employee contributions to the plan will be deposited each pay period. Employees who have completed one year of employment, and have worked a minimum of 1,000 hours, are eligible for the employer’s matching contribution – up to 5% into the plan. Eligible employees may enroll for participation in the plan on January 1 or July 1 following their one-year anniversary date. The Agency will contribute an equal amount of the employees’ contribution up to 5% to a tax sheltered annuity plan with Mutual of America Life Insurance Company. This is additional income.

Disability Insurance--All employees that are hired to work 30 or more hours per week, are eligible for group disability insurance coverage through EMC National Life Company. The Agency pays 50% of the monthly premium. Benefits are paid after 60 calendar days of total disability – 60% of insured's income. Benefits are coordinated with Workers' Compensation and Social Security Disability benefits. Coverage is effective the first day of the month following the employee's start date. A waiver must be signed if you decline this benefit.

Credit Union – Dakotaland Federal Credit Union is a full service credit union, offering checking, savings, loans, certificates of deposit, etc. Payroll deduction is available to employees (with the exception of Relief employees) who wish to contribute to savings and/or checking or to pay loans at the credit union. Employees may sign up any time.

AFLAC -- Benefits may be applied for at any time through AFLAC. Employees (with the exception of Relief employees) receive a group rate on the premiums, which are paid through payroll deduction. If interested, please contact Heidi Alfson 996-0940or 999-6248.

Voluntary Dental Insurance -- Dental insurance is available to all employees who are hired to work 30 or more hours per week, and their families through Guardian Life Insurance. Premiums are paid by the employee through payroll deductions. Coverage is effective on the first day of the month following the employee's start date. Benefits include:

Unit 1 -Preventive Services paid at 100% (no deductible) - checkup, cleaning every 6 months.

Unit 2 -Basic services paid at 80% ($50 calendar year deductible applies) - fillings, extractions, root canals, oral surgery, periodontal prophy, periodontal scaling and root planning.

Unit 3-Major Procedures – Inlays, crowns, etc. Call Guardian if you have a question pertaining to benefits BEFORE you have dental work done. Maximum Benefit - $1250 annually.

COST: Single Coverage $22.97 per paycheck

Single + Spouse $42.75 per paycheck

Single + Child(ren) $50.26 per paycheck

Single + Family Coverage $70.04 per paycheck

Fitness Center DiscountsTrue Fitness gives a 20% discount to LifeQuest employees. No annual membership*auto deduction from checking. AnyTime Fitness gives LifeQuest employees a 15% discount on a membership and 50% off of the enrollment fees.

Pre-Paid Legal Services/ID Theft - - Legal Benefit Consultants, Inc. provide ID Theft and/or Pre-Paid Legal Services to LifeQuest employees purchasing the services. See Human Resources Manager for costs.

Vision Insurance Avesis Vision Insurance is available to employees who are hired to work 30 hours or more.
