APPLIES TO: All employees of Northpointe Behavioral Healthcare Systems that are eligible for insurance coverage.

POLICY:

It is the policy of Northpointe Behavioral Healthcare Systems that an eligible employee (as defined above) can continue health and life insurance premiums when he/she is on inactive payroll status on the first day of a month. Northpointe shall have no obligation or liability whatsoever for making any insurance premium payments for employees or their lawful dependents beyond the end of the month.

PURPOSE:

To establish policies and procedures regarding continuation of insurance premiums.

DEFINITIONS:

Inactive payroll status: When an employee is not being paid for work performed for Northpointe, i.e., not receiving a paycheck. This includes unpaid leave of absences, worker’s compensation leave and any other unpaid absence, but does not include FMLA leave. This also includes when an employee quits, retires, is laid off or if the employee’s employment is terminated.

PROCEDURES:

A. Health Insurance

1. An employee is eligible to COBRA their health insurance benefits on the first day of the month (as defined above).

2. Northpointe will resume payment of their portion of the health insurance premiums for eligible employees who return to active payroll status as of the first (1st) day of the premium month following the date of the employee's return to work.

B. Life and Long-Term Disability Insurance

An employee can continue these benefits if the employee continues full and timely payment. An employee on unpaid leave may purchase a conversion life insurance policy when he/she quits, retires, is laid off or if the employee’s employment is terminated.

C. AFLAC

AFLAC insurances are individually owned policies. Accordingly, when an employee is on inactive payroll status, the employee must make direct payment to AFLAC to continue their insurance benefits.

D. Prior to discontinuation of health insurance coverage, an employee shall receive notification from Northpointe's Health Insurance carrier regarding the option to elect COBRA and the instructions to follow for COBRA premiums.
