General Responsibilities

Under the direction of the BIH Coordinator and the MPCAH Supervising Behavioral Health Clinician, this position provides mental health brief therapy interventions, case management, and consultation to the Black Infant Health program. This position will promote community mental health interventions; consultation to a multidisciplinary team of home visiting professionals and paraprofessional staff specific to address mental health and other health care needs and service delivery to high-risk clients of the Black Infant Health program; provides inter-professional consultation; conducts and plans educational activities; support joint case conferences, consultation, training and support to the Black Infant Health Program including maximizing quality health care services to children, mothers and families; provides resource development for both targeted high risk populations and program eligible populations through social work consultation, inter-agency coordination and staff training.

This position coordinates with various entities within the Alameda County Public Health Department as well as the community to maximize quality health care services to vulnerable children, mothers and families; provides resource development for both targeted high risk populations and program eligible populations through professional clinical/medical social work consultation, inter-agency coordination and staff and provider training, This position requires the ability to be a team player with multidisciplinary team of diverse staff members; sensitivity to cultural diversity, and a strong ability to support emotionally charged case scenarios while home visiting in complex communities. This position works in coordination with other Mental Health Clinicians and as a team.

This SPMP position requires knowledge of early childhood mental health, social, emotional and behavioral aspects of mental health; trauma informed care; typical and atypical child development, reflective practice; maternal child health; knowledge of pertinent public welfare legislation and functions of county social services agencies to the prenatal and postpartum population to ensure access and services for this high-risk population; as well as, case management and other techniques of therapy with families, couples, groups and individuals, including young children and community resources in the field of mental health.

Specific Duties

Under the general direction, the Medical Social Worker/Mental Health Professional is responsible for the following activities with a particular focus on Medi-Cal eligible and Medi-Cal insured children and families in the Family Support/Home Visiting System of Care, Black Infant Health program:

1. Provides mental health, trauma informed interventions including meeting individually and in a group format with African American high risk pregnant, postpartum and women with young children who are primarily Medi-Cal Eligible. This includes a caseload of 20-25.

2. Provides regular mental health consultation and technical assistance to the Black Infant Health program. This time includes participating in joint trainings, team meetings and case conferences for these programs.

3. Understands and respects the values and beliefs of African American women and the African American community;

4. Fully embrace and support the BIH Governing Concepts of cultural competence, client-centered, strength-based, and utilization of cognitive skill building approaches (details above in the Governing Concepts section)

5. Conduct the client enrollment which includes an orientation, informed consent, and the initial assessment, and refers participant to FHAs for on-going case management;

6. Responsible for conducting case conferencing with local BIH staff and any other members of a multidisciplinary team

7. Develop and maintain relationships with local mental health professionals for client referrals;

8. Participate in the group sessions that focus on mental health issues by being available to answer participant questions and provide support to the group facilitators.

9. Provide medical health education to clients when requested by the FHAs.

10. Conduct trainings on the basics of maternal and infant mental health;

11. Attends CDPH/MPCAH-sponsored BIH Basic Training, as well as subsequent BIH Advanced Trainings;

12. Maintains confidentiality and adheres to HIPAA regulations;

13. Develop a Professional Development Plan in conjunction with the BIH Coordinator and reflects basic skills of the HRSA MCH Bureau Leadership Competencies 1-8;

14. Joins with existing teams/programs and learns current models of service delivery, while maintaining an ability to adapt to changing evolution of Home visiting Integration process.

15. Prepares case reports; documents treatment plans, progress notes and treatment summary related information as required by the program; prepares correspondence regarding social evaluation of client, client follow up and related matters; keeps required statistics.

16. Participates in the Multidisciplinary Team for the Home Visiting Family Support System of Care which will include offering joint case conferencing, training, technical assistance and consultation regarding substance abuse related or mental health related issues to all home visiting programs for best practice skill building and enhanced client services.

17. Monitors and implements perinatal/interconceptional mental health best practices based on current trends and latest research.

18. Provides inter-agency coordination to assure appropriate service utilization and avoid duplication of services.

19. Follows standard protocols, policies and procedures for mental health related services for all home visitors in the Home Visiting System of Care.

20. Develop linkages and studies the gaps in mental health services for pregnant women and new mothers.

21. Promote improved collaboration and education of home visiting agencies serving high-risk pregnant, postpartum and interconceptional women.

22. Other duties as assigned

Knowledge of the following:

• Women’s health, including prenatal and postpartum mental health and psychosocial issues;

• Life course perspective

• Impact of psychosocial, cultural and economic factors on the health of women, their infants and their families;

• Psychosocial risk status of client, including stress, health, social relationship, environment (housing), financial status, transportation that may impact her pregnancy or participation in the BIH Program;

• Prevailing trends and policies in mental health, public health and public welfare;

• Knowledge, understanding and location of community services, including, but not limited to: social services, mental health, substance abuse treatment programs, domestic violence programs, legal systems, housing and other resources for referral;

Demonstrates:

• Cultural competence and ability to operate in a culturally-affirming manner;

• Clinical experience in individual and group psychotherapies dealing with individuals in multiple systems of health and social systems

• The ability to identify behavioral tendencies that impact functioning, and recommend cognitive behavioral approaches that integrate client strengths into improving functioning

• The ability to conduct crisis intervention as needed or as resources allow;

• Sound communication and interpersonal skills;

• Critical thinking and problem solving skills;

• Basic administrative skills (i.e. appointment scheduling, documentation, report writing, computer skills).
