Dear Parent/Guardian(s),Based on the House Bill 7165, the School Readiness program requires that all children receiving School Readiness funds birth-five years old be screened for their growth and development. The purpose of the screening is to ensure your child’s developmental progress will be tracked and when necessary proper follow- up will be offered.As you are participating in the School Readiness Program, the Early Learning Coalition of Palm Beach County, Inc. is providing a developmental screening service for you and your child. Because your child’s first five years of life are crucial for his/her development, we want to help provide the best start for your child. As part of this service, we provide the Ages and Stages Questionnaire (ASQ-3), which tracks your child’s cognitive development, as well as the Ages and Stages Questionnaire (ASQ-SE) to track your child’s social emotional development. While completing the questionnaires, you will be asked to answer questions about your child’s skill level. The questionnaires include questions about your child’s language, gross motor, fine motor, problem solving, and personal-social skills. To screen your child, please sign the enclosed Consent Form.Please see the attached Ages & Stages Questionnaires (ASQ-3 and ASQ-SE), take a few minutes to complete both questionnaires. Please consult with your child care provider if you have any questions or need assistance in completing the ASQ-3 and ASQ-SE. Upon completion of the ASQ-3 and ASQ-SE questionnaires, please return them with your Consent Form to your child care provider.
