Every member has struggled at some point in their recovery with navigating the behavioral health system. It is not easy to get answers to our questions about AHCCCS Medical Assistance, Nutrition Assistance, or Freedom-To-Work programs. What we need to know is now available online, and this flyer provides the links to three important web-sites to help you submit an application, and then manage your benefits effectively. Each web-site requires that you establish an online account by creating a unique user name and password before you can navigate their site. You will also need your DES Case Number, AHCCCS ID Number, Date of Birth, and Social Security Number.
You can use the flyer as a worksheet to create your user name and password so that you will remember it the next time that you logon to these web-sites.
If you have not used a computer before, then ask your case manager or peer specialist to help walk you through the online application process. With a little practice you will be able to skip the long waiting lines, and have all the information you need to obtain and manage your benefits. You can check your eligibility renewal date so that you can alert your case manager and provider agency, and avoid being dropped from AHCCCS!
These web-sites can also be accessed from other health care provider organizations, and even through computers available to the public at the Flagstaff Public Library.
