1. PURPOSE. This bulletin provides VA facilities and other interested organization with information concerning the mailing addresses of VA locations and jurisdictional areas assigned to regional offices, outpatient clinics, Memorial Service Networks, and the Health Revenue Center.

2. GENERAL (this Bulletin is located on the VA Intranet at vaww.ofcadmin):

a. This issue of the Consolidated Address and Territorial Bulletin lists all the necessary information formerly contained in the Consolidated Address and Territorial Bulletin 1-M.

b. When using an overnight express mail service for Express-to-Address-Express Mail Service (excluding P.O. Box Numbers) the direct delivery address, along with the name and telephone number of a contact person should be shown. Express mail mailed on Friday which cannot be delivered to the address on Saturday will be delivered the next business day. Questions concerning the availability express mail to a particular city should be directed to local express mail Customer Service Representatives. This issue provides the two-letter State abbreviations which may be used in conjunction wit the Zip Codes. The use of two-letter State abbreviations is not mandatory. Information concerning the addresses, facility numbers, servicing facilities and Memorial Service Networks for National Cemetery Administration field facilities is included in Section V. VA facility telephone numbers may be found in local telephone directories, the Department of Veterans Affairs Central Office Telephone Directory and on the VA Intranet at vaww.sta/guide/home.asp.

c. Changes to any information in this bulletin should be forwarded to the Chief, Building Management Division (033A); 810 Vermont Avenue, NW, Washington, DC 20420.

d. For reference purposes, the three-digit facility numbers are included in Section IV.

e. Those facilities assigned station numbers are listed by State in alphabetical order in Section III. Those activities not assigned numbers and under the jurisdiction of field facilities are indented and listed

alphabetically following the parent facility. Other activities not assigned numbers are under jurisdiction of Central Office but are listed alphabetically by State in which the field activity is located.

f. Where a facility with Regional Office activities has VBA jurisdiction for an entire State, the Footnote column of Section III will show a "1". Outpatient clinics having jurisdiction for an entire State will

show a "3"and the variable jurisdiction will be shown in Section VI. If the outpatient clinic has partial jurisdiction for a State, the Footnote column will show a "4" and the variable jurisdiction will be shown in Section VII.

g. In general, a facility is a field installation or group of installations under a single Director. A complex of two Medical Centers or a Regional Office and a Medical Center under a single Director is a

single facility for purposes of the numbering system. For Statistical purposes, such installation are counted separately, as separate Medical Centers or as a separate Medical Center and Regional Office;

for example, the complex of two Medical Centers at Palo Alto is counted as separated medical centers The Palo Alto Division and the Menlo Park Division.

h. The following abbreviations for Type of Activity are used in this bulletin under the heading "Symbol":
