CCCR Improving Minority Health on Rheumatic Diseases

Methodology Core (PI: Nietert/Wolf)

The goal of this MCRC is the advancement of knowledge with respect to African Americans who have, or who are at risk of developing, systemic lupus erythematosus, systemic sclerosis, and other debilitating rheumatic diseases. The overall objective of this Core is to provide rigorous methodological and biostatistical support to the MCRC investigators and to lead investigations into racial/ethnic components of rheumatic disease that are focused in methodological areas including studies of gene x gene and gene x environment interactions.

Blink Reflexometer

Goals: The blink reflexometer device is a proof of concept prototype to explore the ability to create a device that could capture and analyze spontaneous and stimulated blinks. It has proven that blinks can be captured with good repeatability. The next step will include device refinements to support a series of testing and to bring the technology closer to a “field-deployable” device. The initial numbers stated are to gather preliminary results and statistical significance to determine if the number of subjects needs to increase.

Glycoshpingolipids as Therapeutic and Biomarkers of Lupus Nephritis

Goal: Lupus is a chronic autoimmune disease and the most severe complication is lupus nephritis (LN), which affects up to two-thirds of lupus patients. While mediators of LN remain largely unknown, its features are common to other chronic kidney diseases that are mediated by dysfunctional lipid metabolism. The goals of proposed studies are to identify mechanisms that cause dysfunction of this lipid pathway in kidney cells and identify how dysfunction of this lipid pathway affects progression of kidney disease using a relevant lupus mouse model of chronic kidney disease. The proposed studies will also identify additional targets for early intervention thus relieving the burden of increased health care costs for these patients and improving their quality of life.

Role of pericytes in scleroderma skin and lung fibrosis

There are currently no effective therapies to treat existing lung fibrosis; the only option for patients is organ transplantation. We have identified a peptide from the carboxy-terminal region of endostatin (E4) that exerts anti-fibrotic properties in mice and in human model systems[pic], allowing us to investigate its mechanisms of anti-fibrotic action. We hypothesize that activated pericytes contribute to myofibroblast populations in both skin and lungs, and pericytes are an important target cell for the anti-fibrotic activities of E4. To investigate these hypotheses, we propose to (1) compare the role of lung and skin pericytes as myofibroblast progenitors in murine models of fibrosis, (2) compare the distribution and function of pericytes from normal and fibrotic lung and skin from scleroderma patients, and (3) determine if skin and lung pericytes are a target of the novel anti-fibrotic peptide E4.

Predictive Biomarkers for disease activity and organ damage in patients with lupus

The goal of the proposed research is to examine the diagnostic and prognostic capabilities of two or more statistical models developed from multiple urine biomarkers as tools to reliably identify a subject’s response to induction therapies for lupus nephritis and to predict nephritis flares.

Urine biomarkers of lupus nephritis pathology and response to therapy

The objective of this proposal is to identify noninvasive urine protein markers of disease that are indicative of renal pathology and response to therapy in lupus nephritis. Multiple low abundance proteins are also being analyzed. To increase predictive power in this heterogeneous disease, artificial neural network modeling is being used to create diagnostic and predictive models of disease and response to therapy.
