SCOPE OF WORKGENERAL SCOPE This Addendum describes changes to the scope for Project 528A5-14 -516 to furnish and install dock levelers in Buildings 10 and 11 at the VA Medical Center 400 Fort Hill Avenue, Canandaigua NY 14424. This document provides an overview of the work and other pertinent items affecting the scope of work. The plans, specifications, existing conditions and balance of the Contract Documents must be consulted for the entire project content.DELETED WORKAll work related to the three dock levelers at Building 10 are deleted from the project. NEW / ADDITIONAL WORKBuilding 11 Installation and preparatory work required for installation including concrete replacement requirements, electrical requirements and installation details.EXISTING CONDITIONS Building 11 - Occupied during normal business hours. Work shall occur during regular VA working hours Monday through Friday 8:00 AM to 4:30 PM. Logistics – The contractor shall provide storage for all items related to the work in locations outside of all VA structures. Building 11 shall not be used to receive or store items related to the work. The VA will not receive or store items related to the work. FURNISH AND INSTALL The Contractor shall provide renovations / repairs to existing dock levelers as described and adjacent service areas as described in the Contract Documents including the Plans, Specifications and Project Scope Documents. The Work is to include but not be limited to the following overview:Provide Site Demolition per drawings and required for new work, and remove all debris from the site and dispose within codes and regulations.Isolate the work area from access by non authorized VA personnel and Veteran populations. Provide temporary building utility services during shut down conditions necessary for construction. Service shut downs are allowed only during VAMC non-working hours and contingent on approval by the COR.All equipment shall be designed and installed to achieve effective mechanical service and ease of access and maintenance with conformance to spare parts and standard equipment systems in use at the Canandaigua VA facility.The contractor shall use dust mitigation procedures to limit particulates in the area of work. This includes sealing potentially impacted areas and using wet sawing / cutting practices. The Contractor shall perform site inspections prior to the bid and include all work necessary to identify service routing to suit field conditions and complete the work. DetailsDetails and Sections528A514-416D2Pit Details Specifications: Included in this addendum are the following added specifications:Division 01General RequirementsDivision 01CommissioningDivision 26ElectricalIn addition to the listed specifications the following shall be included:DIVISION 1 GENERAL REQUIREMENTSThe Contractor shall perform site inspections prior to the bid and include all work necessary to identify service routing to suit field conditions and complete the work.All equipment shall be designed and installed to achieve effective mechanical service, ease of access and maintenance with conformance to spare parts and standard equipment systems in use at the Canandaigua VA facility. Use the VA recommended items list for components and equipment. Provide general renovations for architectural features and finishes including but not limited to architectural features, electrical, mechanical, paint, acoustic ceilings, floors, plaster, doors and hardware. Replace and upgrade existing finishes. Use ADA approved hardware, fixtures and appliances.Provide temporary building utility services during shut down conditions necessary for construction.Shop Drawings - The contractor shall update weekly a submittal schedule on an Excel spreadsheet conveyed to the COR. Prior to mobilization the spreadsheet shall be submitted containing a listing of all items for subsequent submittal. The VA shall have 20 days to review and return all submittals. Work shall not proceed until all material has been approved and available for construction.All concealed roughing shall be reviewed by the VA for conformance to Codes and Specification prior to proceeding with coving or embedment in contiguous material.Applications for payment shall be on a formal Earned Value basis. There shall be a one to one line item correlation between the schedule and the schedule of values. The project is classified Type IV with required control measures identified on Attachment A Infection Control / Industrial Hygiene Risk Assessment and Permit. The contractor shall include for VA review separate written plans for Site Specific Infection Control and Site Specific Safety - HSE. The plans shall identify and address all risks and issues of the work and control measures identified on Attachment A 600-00C-26I-05 ICRA. General references to codes will not be acceptable. Only measures specific to the site shall be acceptable. Provide HEPA filtered negative air in project spaces throughout the workThe contractor shall provide the means and methods to fully comply with construction related Site Safety requirements and Infection Control procedures.In addition to all other governing standards the following shall be included as pertinent to assembling the Site Specific Plans:JCAHO E-edition EC.02.06.05JCAHO CAMAC - Section 1, Surveillance, Prevention and control of Infection - Standard EC.5.50 and EC.8.30The contractor’s Project Manager shall hold weekly status and coordination meetings with the COR at a site location designated by the VA.Payment for common items of materials or items stored on site shall not be allowed until installed.Remainder per drawings and specifications.DIVISION 3 – CONCRETE4000 PSI concrete, ready mix from commercial batch plant shall be used.Vertical exposed surfaces shall be hand rubbed with a cementitious mixture upon removal of the form work. Finish shall be smooth and free of voids.Horizontal exposed surfaces shall be a steel trowel finish. Provide weather and temperature protection and curing methods as recommended by AIC guidelines.Remainder per drawings and specifications.DIVISION 26 – ELECTRICALConduit shall be a minimum of ?”, GRS or EMT. Use GRS conduit for all exterior power applications.Conceal metal conduit for electrical power in slab from interior of B11 to dock leveler service connections. Use ridged all metal conduit in B-11.
