Become an instructor for the premier public safety cyclist training organization.

Since 1991, IPMBA Instructors have educated countless public safety personnel in using their bikes safely and effectively on the job. This intense course will increase your confidence, reinforce your understanding of the basic principles of public safety cycling, improve your bikehandling skills, and prepare you to communicate your knowledge to others. Teaching methods include in-class presentations, on-bike drills, and peer and self-critiques. You will learn to identify and correct improper technique; how to assist students in overcoming individual difficulties; and how to incorporate various methods of instruction into your courses. When you have completed this course, you will be equipped with the skills you need to teach beginner and intermediate cyclists how to use the bike, not just as transportation, but as an indispensable tool for success on the job. Certification will be issued upon successful completion of the course requirements.

Important Information! Please Read First! In order to be eligible for the IPMBA Instructor Course, you must submit the following items: 1) completed IPMBA Instructor Application/Cycling Experience Questionnaire (included in this packet); 2) signed Letter of Expectations (included in this packet); 3) signed letter of departmental support (included in this packet); 4) Instructor Recommendation Form (included in this packet; required for non-IPMBA-trained candidates; recommended for IPMBA-trained candidates); 5) this registration form; and 6) all applicable fees.

Note: Applications are subject to review. We recommend that you wait for approval before purchasing non-refundable travel. If your application is incomplete or you do not meet the pre-requisites described in the application, your course registration fee ($600) will be returned. The application ($125), membership ($60), and basic certification ($15) fees are non-refundable. You will be notified of your acceptance into the course via email within three weeks of receipt of your completed application. All paperwork and applicable fees must be received by October 5, 2018. Spaces cannot be reserved without payment or application approval. No Walk-Ins.

If you are dismissed from the class for not meeting the standards described in the Letter of Expectations, your course registration fee will not be refunded.

The registration deadline for this class is October 5, 2018. The IPMBA Office will be closed September 17-October 5. Applications and registrations received during this time will be processed on October 8.

TRAINING LOCATION The course will be held at the Arizona State University Police Department, 325 E Apache Blvd., Tempe, AZ 85287-1812. The training location is approximately one mile from the Holiday Inn Express in Tempe.

AIRPORT TRANSPORTATION The Holiday Inn Express Phoenix-TempeUniversity is five miles from Phoenix Sky Harbor Airport and 27 miles from Phoenix-Mesa Gateway airport (Allegiant Air). The hotel has a free shuttle to Sky Harbor but NOT to Gateway. The shuttle cannot accommodate bicycles.

BORROW A BIKE! ASU Police has more than 30 department bicycles, all in good working order. They will be made available on a first-come, firstserved basis. Contact Katie Fuchtman at katie.fuchtman@asu.edu to reserve a bike in your size.

The bicycles have toe clips (cages and straps; bring your own pedals if you prefer a different style of pedal retention), kickstands, racks, and rack bags. You must bring your own headlight, and EMS personnel must provide their own panniers.

BIKE SHIPPING If you prefer to bring your department bicycle or would like to bring a personal road or mountain bike for recreational rides, you may ship it to the ASU Police. The ASU Bike Co-op (http:// fitness.asu.edu/services/bike-co-op) will assemble and disassemble bikes at no cost. Bikes must arrive no later than October 22 to allow ample time for assembly and repairs, if needed.

Instructor Materials: Two presentations (mock teaching sessions, 10 and 15 minutes) are required during the class. If you are comfortable using certain teaching or visual aids (flipcharts, PPT, etc.), please bring them or come prepared to purchase them. You will be required to use two or more different visual aids during your presentations, and PowerPoint can only be used in one of them. Projection equipment and flipcharts will be available for in-classroom use only, not for practice or preparation outside of the classroom.

CLASS SCHEDULE The full training schedule will be distributed with the student materials on the first day of the course. The course hours are 0800-1800 MondayTuesday; 1300-2300 Wednesday; 0900-1800 Thursday; 0700-1500 Friday. Note: schedule may be changed at the discretion of the instructors.

Parking at ASU costs $8 per day in a lot is adjacent to the police station. The hotel and the department are only one mile apart, so biking is recommended.

RECREATIONAL RIDES Mountain and road riding opportunities are numerous. Current ASU instructors will be provide information on these rides and will accompany persons who are interested in recreational rides during off-hours.

WEATHER Daytime high temperature averages 83 degrees and nighttime lows average 54 degrees. Apply sunscreen liberally.

(A Questionnaire for People Aged 15 to 69)

Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active.

If you are planning to become much more physically active than you are now, start by answering the seven questions in the box below. If you are between the ages of 15 and 69, the PAR-Q will tell you if you should check with your doctor before you start. If you are over 69 years of age, and you are not used to being very active, check with your doctor.

Common sense is your best guide when you answer these questions. Please read the questions carefully and answer each one honestly: check YES or NO.

YES NO 1. Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?

2. Do you feel pain in your chest when you do physical activity?

3. In the past month, have you had chest pain when you were not doing physical activity?

4. Do you lose your balance because of dizziness or do you ever lose consciousness?

5. Do you have a bone or joint problem (for example, back, knee or hip) that could be made worse by a change in your physical activity?

6. Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?

7. Do you know of any other reason why you should not do physical activity?

If you answered

YES to one or more questions

Talk with your doctor by phone or in person BEFORE you start becoming much more physically active or BEFORE you have a fitness appraisal. Tell your doctor about the PAR-Q and which questions you answered YES.

? You may be able to do any activity you want -- as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice.

? Find out which community programs are safe and helpful for you.

NO to all questions

If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that you can: ? start becoming much more physically active ? begin slowly and build up gradually. This is the

safest and easiest way to go.

DELAY BECOMING MUCH MORE ACTIVE: ? if you are not feeling well because of a temporary illness such as

a cold or a fever ? wait until you feel better; or ? if you are or may be pregnant ? talk to your doctor before you

start becoming more active.

? take part in a fitness appraisal ? this is an excellent way to determine your basic fitness so that you can plan the best way for you to live actively. It is also highly recommended that you have your blood pressure evaluated. If your reading is over 144/94, talk with your doctor before you start becoming much more physically active.

PLEASE NOTE: If your health changes so that you then answer YES to any of the above questions, tell your fitness or health professional. Ask whether you should change your physical activity plan.
