NHS TAYSIDE/NHS FIFELOCUM CONSULTANT VASCULAR SURGEON JOB DESCRIPTION (STARTING JUNE 2020)THE DEPARTMENTSVascular services for NHS Tayside and NHS Fife have been delivered through a network, East of Scotland Vascular Network (ESVN), since 2002. This network is staffed by 6 full time NHS Consultant surgeons and 1 Specialty Doctor working between Ninewells Hospital and Medical School, Dundee (NWH), the Victoria Hospital, Kirkcaldy (VHK), Queen Margaret Hospital, Dunfermline (QMH) Perth Royal Infirmary, Perth (PRI) and Stracathro Hospital. The vascular service is supported by 24 hour access to Interventional Radiology (IR) based at Ninewells Hospital and to elective daytime IR at both NWH and VHK. Both sites have 24 hour support from Surgical Trainees (ST), Core Trainees (CT), Clinical Fellows and FY1 and FY2 doctors. Each hospital has fully equipped vascular laboratories with vascular technologists employed within the network. In addition the service benefits from close working relationship with the department of Vascular Medicine. All emergency vascular referrals are received at NWH. At present, elective vascular surgery is undertaken mainly in both NWH with some in VHK (under active review). The Ninewells unit consists of 24 beds in Ward 12 and access to 10 high dependency unit beds. There are 10 beds on ward 19 for amputation rehabilitation and limb fitting. The combined population for the network is 800,000.The ESVN carries out a wide range of open and endovascular surgical procedures. There is an established EVAR service deploying fenestrated, branched, thoracic and infrarenal stent grafts jointly with IR. Renal access surgery, surveillance and maintenance are well developed. Amputation rehabilitation and limb fitting are supported by a dedicated physiotherapy team. The majority of varicose veins are treated by day case endovenous procedures. The Network has a strong commitment to both undergraduate and postgraduate training.THE POSTApplications are invited for the post of a Locum Consultant Vascular Surgeon appointment to the East of Scotland Vascular Network. This post is for up to 12 months in the first instance.The ESVN services a hub (NWH) and spoke (VHK/PRI/QMH/Stracathro) model, with the whole team taking responsibility for the Hub and groups of colleagues taking responsibility for their spokes (with cross cover from the whole team). This post is to contribute to the NWH Hub (on call service and main theatre sessions for Tier 2/3/4 work) and to service the NHS Fife spoke (Out patients/Tier 1 & 2 surgery).The appointee will be contracted to a standard NHS 8 PA + 2 SPA job, totaling 10 sessions.ConsultantsMr. M. Lees Clinical DutiesThe appointment will take part in the on-call rota, this being on the basis of one in six with prospective cover for holidays and study leave. The emergency cover will be for vascular surgical conditions only. Sessions will be based mainly in NWH, with the potential for sessions in VHK, QMH , PRI and Stracathro hospital. The appointee will be expected to work flexibly within the current arrangements. This will involve performing clinical sessions in both Tayside and in Fife. The success of ESVN is underpinned by a close, flexible and open working relationship between its consultants. It is essential that the appointee is able to work within the current modus operandi. The current post holder contributes to the Networks Renal Access and it would desirable if the appointee was able to continue this service.With the addition of a mobile C-arm in Vascular Theatre, there is the capability to perform hybrid/endovascular work out with the IR suite, in conjunction with IR. Our unique working relationship with our IR colleagues is important for our unit and it is imperative that it is maintained.SPAThe appointee will have two SPA sessions timetabled per week.TeachingThe department takes an active role in both undergraduate and postgraduate teaching. It is expected that the appointee will take an interest in the delivery of the units teaching. Research and AuditThe vascular unit provides a supportive environment for both undergraduate and postgraduate research. The appointee will be expected to contribute to ongoing research and quality improvement projects and be involved in the initiation of new projects. The unit actively participates in the National Vascular Registry.Radiology ServiceESVN is supported by close working relationship with X-ray Department in both NWH and VHK providing digital subtraction angiography, ultrasound, CT, MRA and PET scanning. The interventional radiology service offers a full range of peripheral catheter interventions.Management StructureThe Vascular Unit operates within the Directorate of General Surgery. The person appointed will be given assistance from the Clinical Lead for the Vascular Surgery. The line of responsibility passes from the Clinical Lead to the Clinical Director for Surgery who reports to the Associate Medical Director and ultimately to the Medical Director.JOB PLAN OUTLINEMONTUESWEDSTHURSFRISATSUNAMTheatreAdminClinicSPASPAPMTheatreMDMThe above is for purpose of illustration and demonstrates a typical non on-call week. The final job plan will be negotiated and confirmed on appointment. The current rota works on a six week cycle and main theatres can be on any day. On-call is on a one in six frequency with prospective cover, with a surgeon of the week. The on-call surgeon provides ward care for all inpatients during that week in addition to attending emergencies. The current on-call structure involves a weekend (FRI/SAT/SUN) day and night followed by MON-THURS 08:00-19:00. The weekday nights are covered by the remaining consultants who will have no direct clinical care sessions allocated the next day. The appointee may be expected to provide the outpatient services and Daycase operating to the VHK/QMH spoke along with colleagues. Theatre sessions will be allocated for major operating within NWH.The duties outlined in this job plan serve as an indication of the range of duties the postholder will be required to carry out.
