GENERAL GUILDELINES

• Temporary housing to accommodate Veterans with dependents.

• Housing will be provided on a month to month basis, not to exceed twenty four (24) months.

• Residents will be expected to pay a monthly housing fee, which is determined by the length of stay. In extreme financial hardship, the Department of Veterans’ Affairs may waive the housing fee for Veterans who complete the agency waiver form and provide proper documentation verifying an income hardship.

• Veterans in the program must be capable and willing to live independently and agree to enroll in college or trade school, be employed and/or volunteer in a structured program while a resident.

• Veterans will be assigned a Case Manager and will be expected to develop and follow an Individual Recovery plan (IRP). A minimum amount of structured time will be devoted to employment search, education, entitlements and housing search to achieve the goal of independent living.

• Veterans agree to periodic scheduled inspections of individual and shared living spaces by CTDVA staff.

• Residents must agree to comply with all terms and conditions listed in the Patriots Landing Program Agreement, which will be provided upon admission.

PATRIOTS’ LANDING

PROGRAM

Since 1863, the CT Department of Veterans’ Affairs at Rocky Hill has offered a continuum of services designed to assist veterans in need.

The Patriots’ Landing Transitional Housing Program was originally established in September 2010 in response to a growing need to provide transitional housing services to Veterans.

MISSION STATEMENT:

Provide a safe and secure environment for Connecticut Veterans who are homeless or at risk of becoming homeless. Through supportive transitional housing and case management services Veterans will be assisted in obtaining the necessary benefits, skills and resources needed to transition to subsidized, affordable or independent housing.

HOUSING DESCRIPTION:

The Patriots’ Landing Program is located on West St. in Rocky Hill, CT and is part of the CT Department of Veterans’ Affairs facility. There are five separate residential style three (3) bedroom houses that can accommodate Veterans with dependents or Single Veterans. All houses are completely furnished, equipped with kitchen supplies, bedding, furniture, washers & dryers, and full kitchens. All houses are smoke free. The Connecticut Department of Veterans Affairs is responsible for all maintenance and repairs.

GENERAL ELIGIBILITY CRITERIA

• A veteran must meet the criteria of the definition of veteran as defined in Connecticut General Statutes (CGS) Section 27-103. A veteran must have received an Honorable or General Discharge under Honorable Conditions from the Armed Forces of the United States.

• A veteran must meet all other legal and residency requirements as outlined in the Connecticut General Statutes.

• Submit a completed Patriot’s Landing Program application which can be obtained from ctva or by calling the Patriot’s Landing Admission Coordinator at (860) 616-3803.

• An intake interview conducted by CT DVA staff will be required of all program applicants.

• The veteran will provide information regarding medical conditions, behavioral health, legal issues, financial, or substance abuse information.

• All medical care and associated costs are the responsibility of the individual program participant. Veterans must register with the Federal VA Healthcare System, and/or have alternative medical insurance coverage.

SUPPORTIVE SERVICES:

Support Services will be provided through a collaboration between the CT Department of Veterans Affairs’ and Chrysalis Center Inc. All participants will be assigned a Case Manager who will assist with the following:

• Assistance with applying for entitlement benefits

• Preparing individual and family to live independently in the community

• Referrals to additional Community Resources

• Assistance with obtaining permanent housing

• Referrals to Community Housing Resources

• Career Exploration

• Educational Exploration

• Resume Writing, Networking, Interviewing Skills and Employment search assistance

The CT Department of Veterans Affairs’ Office of Advocacy & Assistance will provide assistance with pursuing available Veterans benefits.
