These services work alongside VisionWest’s Community housing service and are part of the wider services offered by VisionWest. VisionWest Community Trust is an award winning organisation that has been offering community-based services to people in West Auckland since the 1980’s. The Trust was formally incorporated as the Friendship Centre Trust in 1988 in response to a growing desire of the Glen Eden Baptist Church to help those in need in the local community. The Trust responded to the needs present in the community and grew to be one of the largest community-based Trusts in West Auckland. VisionWest now delivers community services to communities in Auckland, Waikato, Bay of Plenty and Canterbury. VisionWest Community Services include:Community HousingHome HealthcareEducation and Training Early Childhood EducationCounselling Whanau ServicesBudgeting Foodbank & Op ShopCurtain, Uniform BanksPurpose of the Position:To work directly with whanau and individuals of VisionWest Community Trust Housing and its multiple housing programmes.Engaging and supporting whanau and individuals, proposing/providing individual assessments and plans, connecting whanau and individuals to individual life skill services, advocacy and supports. To assist people who have housing related issues and receive referrals from local churches and networked Community Agencies, Whanau Services (VW), MSD and HNZ. To provide an integrated and holistic approach to care for whanau and individuals in housing need and to enable and provide client access to relevant services. Proactively engage and work with rough sleepers, with a strong focus on providing support to find sustainable housing.To empower whanau and individuals to live independently with an enhanced sense of wellbeing in all areas of their lives. To work within the aims and objectives of VisionWest Community Trust to provide faith based holistic and wrap around services. To meet all Key Performance Indicators (KPIs) required by the multiple Housing Contracts being delivered by Community Housing VisionWest Trust.Relationships:External Work and Income NZ/Ministry of Social Development Housing NZ Child Youth and Family Services PoliceMinistry of EducationFunding agencies Community providers working with people in housing crisis Community providers working with whanau and individuals in trauma and social needsLocal churches Local community groupInternalGeneral Manager HousingRegional ManagerHousing Operations ManagerClinical Team LeaderTeam membersWhanau ServicesProperty and Tenancy Services Staff of Vision West Community Trust Volunteers and staff of supporting churches Key Duties & Responsibilities:Key tasks Responsibilities Key Performance Indicator Assessing of potential whanau and individuals as referred by Vision West, MSD and HNZInitial interviews, in collaboration with team leader and other team members, with prospective whanau and individuals to determine: Housing needs Appropriate housing based on the needs of the whanau Client involvement All engagement and contact recordedSupport needs Assessment of support required by Support Navigator & other servicesAppropriate client action plansAll information and plans recorded and filedInitial Assessment and updates Initial assessment completed and monthly updatesSupported Housing: General Initial contact and commitment to work with client per Housing Contract All whanau and individuals have initial contact within specified days of referral agreementComplete all required engagement documentation All tenants have completed forms and have begun goal plans per the KPI’s of the respective Housing ContractsCarry out assessment and put ongoing goal plans in place Goal plans are reviewed monthly Intervention where necessary Appropriate intervention occurs Engage with whanau and individuals on a regular basis per their needs and contract requirements with timely reviewsEngagement is timely and intentional as required by respective Housing ContractsLiaise with Property and Tenancy Services when appropriate action is required over any issues arising from the tenancy itself e.g. damage to property, issues with neighbours, over crowding A collaborative working relationship with Property and Tenancy Services over tenancy issues and interventionFacilitate tenant involvement with community living, Vision West Services, local community providers and local churches A collaborative working relationship with Vision West Whanau Services to co-ordinate internal services to empower whanau and individualsRefer to, and receive referrals from, WINZ, Midwives, Churches, CABs, HNZ, CYFs, Schools, Community Workers and Social Workers from networked community agencies and individuals Creating and engaging with community networks to assist whanau and individualsAdvocate for whanau and individuals by attending with them meetings with such agencies as, but not limited to: WINZ, Court, Probation, CADS, Health and Mental Health services, ACC, Legal services, CYFS, other social services Advocacy and referrals to appropriate services occur as required in the best interest of whanau and individuals Work alongside tenants in obtaining counselling, budgeting and other self-help groups they may be interested inAssist and when necessary advocate on behalf of tenantsGeneral Administration, Planning and & ReportingKeep database up to dateData base information is current and up to date Maintain statistics and write reports on all aspects of whanau and individual tenancy and engagement as required, meeting accountability requirements Accurate reports and statistics are available as and when required. All meeting notes are entered to database within 48 hours of meeting or soonerAttend to telephone enquiries and correspondence as required. Correspondence is timely Contribute to development of organisational plans, policy and procedure The Community Housing Team and Regional Manager are well supported in their workCarry out evaluation and quality processes as required by the organisation Complete other tasks that are within capabilities as munication and Team Support Meet regularly with the Team Leader for supervision. Regular supervision Take part in team meetings and participate effectively as part of a team. Quality participation in individual and team meetings. Maintain strong links with local churches and work in with their initiatives and programmes that would be of benefit to Housing whanau and individuals Good relationships with relevant bodies are fostered and maintained. Develop and maintain links with other relevant bodies, including government and the community.Participation of Housing whanau and individuals in Vision West programmes and initiativesSupervision and training Identify and undertake suitable training opportunities. Training undertaken with knowledge and skills increased to enhance work capabilitiesAttend external supervision for support in all aspects of the role and in maintaining a personal balanceIntentional selfcare and proactive engagement with all supervision and trainingNetworking Develop and maintain links with relevant bodies including government, non-government, community groups, housing providers, housing action groups. Promote the services of local churches to the target group and the broader community.The work of the trust is promoted appropriately in the community.Person SpecificationsIdeally the Support Navigator will possess the following skills and personal attributes:Qualifications & Experience:A tertiary qualification in Social Practice Services: Social Work, Community Development, Nursing, Occupational Therapy, etc.Experience working with those who have experienced abuse and traumaSkills, Knowledge, Abilities:Demonstrate the ability to work in a bi-cultural context and acknowledge the Treaty of Waitangi to practice.Have the ability to work with individuals, families or whanau, communities and groups from diverse ethnic and cultural backgroundsExperience of providing excellence in customer service with good interpersonal skills to include telephone & face to face interviewsA sound understanding of community service providers, not-for-profit organisations and / or community housing practices or working within a residential or not-for-profit settingAn understanding, ability and desire to work within the culture and values of a faith based, not for profit organisationExcellent interpersonal skills to deal sensitively with clients and work collaboratively with the Board, colleagues, government and other professionals.Excellent written communication skills including letter and report writing.
