Please be sure to complete the student symptom monitoring survey PRIOR to coming to campus for the New Graduate Student Welcome. All UCLA students are required to complete the symptom monitoring survey each day they plan to come to campus for any purpose. Upon completion of the survey, the system will generate a clearance certificate.
