-152400-266700POSITION DESCRIPTIONPOSITION TITLE:Supports Facilitator- Partners in RecoveryREPORTS TO:Line Manager- Mount IsaAUTHORISED BY: DATE:PURPOSEThis position aims to improve system responses and outcomes for people with severe and persistent mental illness and complex needs to sustain and support optimal health and wellbeing and recovery by:Facilitating better care coordination for individual PIR consumers.Identifying and responding to gaps in services.Improving referral pathways and acting on service integration opportunities.Developing and strengthening partnerships and linkages with service agencies.Promoting a community based recovery model.KEY RESPONSIBILITIESProvide care coordination, information and support (including brokerage) to eligible clients under the PIR program to promote seamless service delivery that meets a full range of an individuals work and develop collaborative relationships with relevant community services and government organisations through building service pathways to successfully link clients to relevant services.Work within the policy, procedures, guidelines and work instructions of the Partners in Recovery petently collect qualitative and quantitative data as required using the data management and client information systems provided for the purpose of monitoring, reporting and evaluating the program.Participate in the local community collaborative advisory group meeting to recognise and respond to service gaps.Undertake any other tasks as directed by the line ply with legislation (including Workplace Health and Safety, Privacy and Equal Employment Opportunity), service agreement and Quality Management System requirements.KEY PERFORMANCE INDICATORSAble to identify eligible Partners in Recovery clientsProvision of a service to identified clients within the PIR framework.Engaged with community networks and established service pathwaysWorking relationships developed and maintained with relevant service providersAppropriate data entered into the data management system and the client information system.Attended professional development / training sessions.SELECTION CRITERIAMandatory Selection CriteriaMSC1:A formal qualification relevant to the position. This may include, but is not limited to qualifications in Social Work, Psychology, Mental Health Nursing, Certificate IV in Mental Health or three years of relevant experience in a similar role.MSC2:Ability to possess a Positive Notice for Child Related Employment (Blue Card and a yellow card exemption) from the Public Safety Business Agency (PSBA).MSC3:Possession of C class driver’s licence. Key Selection CriteriaKSC1:Expertise in providing care coordination services to clients in a mental health context.KSC2:An understanding of the recovery model of mental illness and the ability to communicate and work effectively with the identified client group.KSC3:Ability to communicate sensitively and effectively with culturally and linguistically diverse people and in particular Aboriginal and Torres Strait Islander people. KSC4:Demonstrated written and verbal communication skills, including interpersonal skills, conflict resolution, negotiation, collaboration, report writing and case note.KSC5:Knowledge of the North West service area and an understanding of the issues confronting people residing in these diverse communities.
