Its services and innovations are regarded as best-practice, having won the Premiers’ Award for Health Service of the Year in 2010.Website Information: & Values: Strategic Plan: Purpose of PositionAssertive Outreach is a way of organizing and delivering services via a specialized team to provide highly coordinated, client focused services for disengaged individuals who are not well represented in mainstream NSP environments”ISCH MHOS Assertive Outreach staff will be required to undertake assertive outreach activities during the day, and evening in identified hot spots across the municipalities of Port Phillip, Glen Eira, Stonnington, Kingston and Bayside, targeting disengaged injecting drug users and promoting harm minimisation principles to minimise risk of blood borne virus infections. A particular focus will be to engage young people, Aboriginal and culturally diverse populations as well as people experiencing homelessness. The work requirement for the role will require 3 late shifts until 10 pm on a Tuesday, Wednesday and Thursday once a fortnight. This may change in response to service development and client need. A key strategy will be to undertake consultation within ISCHS to develop accessible pathways into ISCHS primary health services for injecting drug users. Assertive Outreach workers will engage with Police and community agencies within each of the 5 LGAs to capture intelligence on local hot spots and developing communities of use. They will provide practical education and advice on safe using practices, management of injecting sites to prevent infection, and wound care. In addition they will promote the use of secondary NSP’s and Safe Dispensing Units, and will hold the Safe Dispensing Unit portfolio of the MHOS service ensuring that ISCHS SDU’s are constantly stocked, and reporting any maintenance issues to the manufacturer for immediate resolution.
