|To undertake comprehensive and holistic assessments of adults with long term conditions and their carers (formal/informal) within the home |

|environment. |

|To be responsible for organising and managing your own caseload; to ensure assessments and reviews are performed and documented in a timely way. |

|To develop good understanding of the important role of equipment, minor and major adaptations and appropriate housing play in enabling and promoting |

|functional independence. Develop understanding of the legislation and criteria that inform our practice. |

|To provide support and input to the OT assessment clinics across the county. |

|To develop competent moving and handling skills and learn techniques to enable single-handed care. |

|To contribute to case resolution through input to the OT Triage function. To gain knowledge and experience of how referrals are managed from the |

|first point of contact and how innovative interventions are used to reduce risk, meet clients’ needs and effectively manage the demands on our |

|service. |

|To develop and consolidate areas of clinical interest, input into project work focused on service development and improvement, audits and reviews, |

|evidence-based practice and raising the profile of community OT within adult social care and wider community. |

|To engage in regular supervision and ongoing professional development; to have good self-awareness and be able to identify own limitations and |

|training needs and apply this to practice. |

|To be responsible for supervising and supporting OTA staff. |
