Welcome/Introductions: Nancy Muzzarelli of Shawnee Health Service welcomed everyone to the meeting and introductions were given of all in attendance.

Nancy informed the committee that Kris Sherrill has changed positions within SIH the first of April and is working in the Process Innovations Department. Her decision to accept this new position and leave Community Benefits was made after the February meeting and she is sorry she cannot be with us today.

Nancy also informed the committee that Woody Thorne, Vice President of Community Affairs at SIH, met with the Coalition Steering Committee and discussed the transition between Kris and her successor. Mr. Thorne foresees some changes in the position with Kris’ replacement taking an active role in the analysis of patient outcome data and overall population health and sees this work as being consistent with the collaborative nature of the F/WHCC. With reforms in health care, community collaboration will be even more essential to bring about Patient Centered Care and improved outcomes. Our coalition already works closely with Bi-County Health Department in assessing the needs of our residents and identifying priority health problems. As these processes move forward, we may decide that we want to modify our Action Teams. The 2012-17 priority health problems have been identified as Access to Care, Chronic Disease, and Behavioral Health.

Projects that fit the mold of “improving outcomes” include the Hospital ED Referrals for Oral Health project, the Bridges program that places a Shawnee Alliance case manager in the hospital setting to assist senior patients and their families connect to community services before they are discharged from the hospital. SIH is also working with the H Group on new efforts to improve services for people with behavioral health and mental health needs.

▪ Navigating the Road Ahead – This complimentary seminar is for seniors entering the retirement years and understanding the options of planning for the future. ▪ Healthy Lifestyles

➢ New Co-Chair – Caleb Nehring announced that Carrie Eldridge will be joining him as the new co-chair of this action team.

➢ Community Leaders Forum for Franklin and Williamson counties. The event will be by invitation only and will be held on October 25, 2012 at 8:30 a.m. at John A. Logan College. The goal of the forum is to educate and encourage decision makers in the community to be mindful of PSE (Policy, System & Environmental) changes to help resident’s live healthier lives.

➢ Colon Cancer “Drive-Thru” take home kits were distributed on March 30th. 386 kits were distributed in a five county region.

▪ Healthy Seniors

➢ Planning continues for the 6th Annual Senior Fun Fest to be held on May 11, 2012 at John A. Logan College. This year’s theme is: “Seniors of the Caribbean.” Tickets for the event sold out in 5 days.

➢ The next Healthy Seniors Action Team meeting is scheduled directly after today’s meeting at 1 p.m.

➢ Access To Care

➢ The combined group of the F/WHCC and JCHCC Access To Care Action Team has been focusing their efforts on Oral Health concerns with increasing access for oral health services in Jackson, Franklin, and Williamson counties.

➢ It was reported that after running the oral health data in the first complete calendar year, 2011, of the pilot program SIH referred 131 patients for emergency dental appointments. Almost 50% of those referred (63) kept their appointment and received a resolution for their oral health concern.

➢ The IFLOSS Coalition meeting was held last month. In the process of preparing a five-year plan.

➢ The Web-based E-Referral Manual for Jackson County and the surrounding region has been updated and is located on the SIH Website and on the Community Health Calendar. This web-based referral manual has information regarding primary care, dental care, mental health services, financial support, housing assistance, wellness and linkage resources, etc. Most resources link directly to an agency’s website so you can be sure you are receiving the most up-to-date information. ➢ CPR Training – John A. Logan College is a CPR Training Center for this area. There is no charge for these classes.

➢ The next focus area for this action team will be “Behavioral Health”.

Agency Presentation: Susan Durfee, RN, BSN, Health Promotion/Disease Prevention Program Manager Veterans Health Education Coordinator gave this month’s agency presentation.

VA Medical Center Mission: Honor America’s Veterans by providing exceptional healthcare that improves their health and well being.

Background information:

➢ Original construction was built in 1942 and is located at 2401 West Main Street, Marion, IL

➢ The Marion VAMC provides care to over 43,722.

➢ Services offered: Inpatient, surgical, behavioral medicine, emergency room, Community Living Center

➢ Nine (9) Community Based Outpatient Clinics

➢ One (1) additional Community Based Outpatient Clinic under construction in Carbondale, IL and is tentatively scheduled to open late fall of 2012 in the old Officemax building.

➢ A new Evansville Clinic opened in December, 2011. It is a 96,000 square foot building with added new services such as a CT scanner and ultrasound machine. It is expected to serve approximately 14,000 veterans, primarily from southwestern Indiana and nearby parts of Kentucky.

➢ Telehealth is a program that is available for caregivers of Veteran’s with dementia.

➢ VIP Program – Veterans Independence Program is a new way of providing Veteran’s with the assistance you need to remain at home. The Marion VAMC has teamed up with local Area Agencies on Aging to bring VIP assistance to Veteran’s. ➢ Women Veterans Health Care – every VA facility is committed to meeting the unique needs of Women Veterans by delivering the highest quality health care to each woman, while offering the privacy, dignity, and sensitivity to gender-specific needs that she deserves. Your local VA facility has a full-time Women Veterans Program Manager to assist in general health, mental health and special programs.

➢ My Health-eVet is the Veterans online program for Veterans to managing their health. By using this online program, Veteran’s can refill VA prescriptions, use secure messaging, view and request e-mail reminders for VA appointments, view chemistry/hematology lab results, view personal wellness reminders, etc. For more information go to: myhealth. or contact Todd Wright at the VA Medical Center at (618) 997-5311, ext. 54381.

➢ Forward Together HISA – For Home Improvement and Structural Alteration, this program will allow Veteran’s with disabilities to receive assistance for home improvements necessary for continuation of treatment or for disability access to the home and essential lavatory and sanitary facilities.

➢ Forward Together AAE – For Automobile Adaptive Equipment, this program provides equipment and training necessary for certain service-connected patients to enter, exit and operate a motor vehicle, once they have been awarded and Automobile Grant by VBA (Form 4502) and completed Driver’s Training with an approved driver’s evaluator.
