64B3-5.0011 Definitions.

(1) “AAB” means The American Association of Bioanalysts.

(2) “ABB” means The American Board of Bioanalysis.

(3) “ABCC” means American Board of Clinical Chemistry.

(4) “ABD” means American Board of Dermatology.

(5) “ABFT” means American Board of Forensic Toxicology.

(6) “ABHES” means The Accrediting Bureau of Health Education Schools.

(7) “ABHI” means The American Board of Histocompatibility and Immunogenetics.

(8) “ABIM” means American Board of Internal Medicine.

(9) “ABMG” means American Board of Medical Genetics.

(25) “CG” means Cytogenetics.

(26) “CHS” means Certified Histocompatibility Specialist.

(27) “CHT” means Certified Histocompatibility Technologist.

(28) “CT” means Cytotechnologist.

(29) “DLM” means Diplomat Laboratory Management.

(30) “ELD” means Embryology Laboratory Director.

(31) “GS” means General Supervisor.

(32) “HCLD” means High Complexity Laboratory Director.

(33) “HT” means Histotechnician.

(34) “HTL” means Histotechnologist.

(35) “MB” means Microbiology.

(36) “MDT” means Molecular Diagnostics Technologist.

(37) “MLT” means Medical Laboratory Technician.

(38) “MLS” means Medical Laboratory Scientist.

(39) “MP” means Molecular Pathology.

(47) “SH” means Specialist in Hematology.

(49) “SMB” means Specialist in Molecular Biology.

(50) “TS” means Technical Supervisor.

(51) “Academic Science” means college level courses in the areas of chemistry, biology, physiology, anatomy, microbiology, immunology, genetics, physics and molecular biology, defined in Rule 64B3-2.003, F.A.C. The term does not include applied science as defined in subsection 64B3-2.003(6), F.A.C.

(52) “Bachelor’s Degree” means a four year baccalaureate degree from a regionally accredited college or university.

(53) “Bachelor’s Degree in Medical Technology” means a four year baccalaureate degree earned at an accredited program, pursuant to subsection 64B3-2.003(9), F.A.C.

(54) “Medical Technology Training Program” means an ABHES, CAAHEP, NAACLS, board approved training program for clinical/medical laboratory scientists or medical technologists, pursuant to subsections 64B3-2.003(9) and (16), F.A.C., or Department of Defense programs that are equivalent to a board approved training program.

(55) “Semester Hour” means one hour of credit in an accredited college or university, pursuant to subsection 64B3-2.003(1), F.A.C., or foreign education equated, pursuant to subsection 64B3-6.002(6), F.A.C.

64B3-5.002 Supervisor.

Qualifications and Responsibilities.

(1) Qualification. Degrees or semester hours of academic credit required in this section shall be obtained at an accredited college or university recognized by the United States Department of Education or by foreign education equated pursuant to subsection 64B3-6.002(6), F.A.C. Degrees not in a chemical or biological science must be reviewed by the board pursuant to subsection 64B3-6.002(8), F.A.C.

(2) To be licensed as a supervisor, an applicant: shall be licensed or meet the requirements for licensure as a technologist; complete a one-hour educational course acceptable to the Board on human immunodeficiency virus and acquired immune deficiency syndrome or provide an affidavit that the course will be completed within six (6) months of licensure; and meet the requirements of one of the options set forth in subsection (3), below:

(e) The Specialist in Chemistry examination administered by ASCP for the specialty of Clinical Chemistry;

(f) The Specialist in Hematology examination administered by ASCP for the specialty of Hematology;

(g) The Certified Histocompatibility examination (CHS) administered by the American Board of Histocompatibility and Immunogenetics (ABHI);

(h) The Specialist in Andrology/Embryology examination administered by the American Board of Bioanalysis;

(i) The Specialist in Molecular Diagnostics examination administered by the American Board of Bioanalysis;

(j) The Generalist Supervisor examination administered by the American Board of Bioanalysis;

(k) The National Registry of Certified Chemists (NRCC) examinations.

64B3-5.003 Technologist.

(1) Technologist Qualifications. Degrees or semester hours of academic credit required in this section shall be obtained at an accredited college or university recognized by the United States Department of Education or, if foreign education, equated pursuant to subsection 64B3-6.002(6), F.A.C. Degrees not in a chemical or biological science must be reviewed by the board pursuant to subsection 64B3-6.002(8), F.A.C. Applicants for technologist licensure in the categories of microbiology, serology/immunology, chemistry, hematology, immunohematology, histocompatibility, blood banking, cytology, cytogenetics, histology, molecular pathology, andrology and embryology shall complete a one hour educational course acceptable to the Board on human immunodeficiency virus and acquired immune deficiency syndrome or provide an affidavit that the course will be completed within six (6) months of licensure.

(2) All applicants for a Technologist license must satisfy the requirements for High Complexity Testing under CLIA Amendments, 42 C.F.R. §493.1489 effective April 24, 1995, which is incorporated by reference and adopted herein and available at or at .

(3) In addition, at least one of the following requirements must be met for specific areas of licensure. In some cases there are multiple options for meeting the requirement.

(a) Microbiology, Serology/Immunology, Clinical Chemistry, Hematology, Immunohematology. A Generalist Technologist license includes the specialties of microbiology, serology/immunology, clinical chemistry, hematology, and immunohematology.

64B3-5.004 Technician.

(1) General Qualifications. Degrees or semester hours of academic credit required in this section shall be obtained at a regionally accredited college or university, or by foreign education equated pursuant to subsection 64B3-6.002(6), F.A.C. In order to be licensed as a laboratory technician, which includes the categories of microbiology, serology/immunology, chemistry, hematology, immunohematology, histology, molecular pathology, andrology and embryology, the applicant shall complete a one hour educational course acceptable to the Board on human immunodeficiency virus and acquired immune deficiency syndrome or provide an affidavit that the course will be completed within six (6) months of licensure.

(2) All applicants for a Technician license must satisfy the requirements for Moderate Complexity Testing under CLIA Amendments, 42 C.F.R. §493.1423 effective January 19, 1993, which is incorporated by reference herein and available at , or . Technicians performing high complexity testing as defined in 42 C.F.R. §493.5 effective April 24, 1995, which is incorporated by reference herein and available at , or and 42 C.F.R. §493.17, effective January 19, 1993, which is incorporated by reference herein and available at , or , and who have been licensed after September 1, 1997, shall meet the minimum educational and training qualifications provided in 42 C.F.R. §493.1489, effective April 24, 1995, which is incorporated by reference herein and available at , or , incorporated herein by reference, including a minimum of an associate degree in laboratory science, medical laboratory technology, or equivalent education and training.

(3) In addition, at least one of the following requirements must be met for specific areas of licensure. In some cases there are multiple options for meeting the requirement.

64B3-5.007 Director; Limitations and Qualifications.

(1) All applicants for a Director license must have the qualifications for a High Complexity Laboratory Director, listed in 42 C.F.R. §493.1443, effective January 24, 2003, which is incorporated by reference and adopted herein, available at fdsys/pkg/CFR-2011-title42-vol5/pdf/CFR-2011-title42-vol5.pdf or at . The applicant shall complete a one hour educational course acceptable to the Board on human immunodeficiency virus and acquired immune deficiency syndrome or provide an affidavit that the course will be completed within six (6) months of licensure.

(2) In addition, at least one of the following requirements must be met for specific areas of licensure. In some cases, there are multiple options for meeting the requirements. Degrees not in a chemical or biological science must be reviewed by the board pursuant to subsection 64B3-6.002(8), F.A.C.

64B3-5.008 Public Health Laboratory Personnel.

(1) Applicants for director level licensure in the category of public health must meet the requirements in rule 64B3-5.007, F.A.C., for licensees at the Director level in chemistry or microbiology.

(2) Applicants for supervisor level licensure in the category of public health must meet the requirements in rule 64B3-5.002, F.A.C., for licensure at the supervisory level.

(3) Applicants for technologist or technician level licensure in the category of public health shall qualify pursuant to the provisions of section 483.812, F.S.

(4) All applicants for licensure as a Public Health Laboratory Scientist shall apply to the Department on Form #DH-MQA 3001 (10/2020) “Public Health Laboratory Scientist Application,” which is incorporated by reference herein, copies of which can be obtained from , or the Board office at 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257 or from its website at .
