Clinical Decision Support for Opiate Use Disorders in Medical Settings: Pilot Usability Testing in an EMR (COMPUTE) (Rossom)

Currently, providers lack real-time on-site clinical decision support (CDS) tools that can facilitate the identification, assessment, and treatment of patients with opiate use disorder (OUD) during a patient encounter. The NIDA CTN Blending Initiative has developed an expert panel framework of an OUD CDS. The framework has been previewed to by primary care providers and at the 2015 INEBRIA conference. It is felt that any further refinements needed are best identified through piloting so that provider acceptance and feasibility of use, including  (e.g., workflow issues,) can be determined. This would be the first attempt to develop and test the feasibility and acceptance of an EMR-based CDS for OUD. Assessing the acceptability and feasibility of using CDS for OUD identification, assessment, and treatment within the primary care setting and comparing this to OUD identification, assessment, and treatment for providers without the CDS will allow for further refinement before the CDS is deployed across multiple sites across the CTN for larger studies of treatment outcome and guideline adherence.

Pragmatic Trial Comparing Telehealth Care and Clinic-based Care for Uncontrolled High Blood Pressure (Hyperlink-3) (Margolis)

The study will compare two approaches to redesign care to help patients with uncontrolled hypertension lower their BP and risk of a future heart attack or stroke. The clinic-based care approach will use recommended best practices in 10 clinics and 1000 patients. It relies primarily on the physician-medical assistant dyad and face-to-face visits. The telehealth care approach will adapt and implement a successful research-tested model in 10 clinics and 1000 patients. It differs from clinic-based care through the systematic use of home BP telemonitoring and home-based telehealth care coordinated by a pharmacist or nurse practitioner (NP).

The objective is to develop and implement point-of-care EHR-based clinical decision support that identifies and prioritizes all available evidence-based treatment options to reduce a patient’s cardiovascular risk (CVR). After developing the EHR-based decision support intervention, we will test its impact on CVR, the components of CVR, in a group randomized trial. This approach, if successful, will improve chronic disease outcomes and reduce CVR for about 35% of the U.S. adult population, maximize the clinical return on the massive investments that are increasingly being made in outpatient EHR systems, and provide a model for how to use EHR technology support to deliver “personalized medicine” in primary care settings.
