Course Learning Outcome(s)

As a result of participating in this learning activity, the learner will be able to:

• Demonstrate knowledge of basic home care documentation principles and practice by passing the post-test with a score of ≥ 80% following the on-line learning activity.

Course Description:

This course is designed to assist therapists and therapy assistants in understanding the unique aspects of documentation in home care. Further, it introduces the many aspects of developing a plan of care from assessment through discharge. Specific home care rules and regulations regarding documentation and staff supervision in this setting will also be included in this course.

Course Objectives:

• Explain common terms, rules and regulations and components in home care documentation and care planning including home bound status and the Outcome and Assessment Information Set (OASIS)

• Discuss the various disciplines/roles in home care, services they may provide and documentation to support the need of these services

• Identify, manage and document rehabilitation potential, DME needs and caregiver support in the home care setting

• Outline necessary documentation related to establishing patient-specific goals and plan of care, special circumstances altering the service plan, discharge planning and Medicare Conditions of Participation requirements

• Describe duties and documentation related to PT supervision of a PTA, OT supervision of a COTA, and supervision of the home health aide

Cindy Krafft PT, MS, HCS-O

Cindy Krafft brings more than 20 years of home health expertise that ranges from direct patient care to operational and management issues. For the past 15 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization and OASIS. She has served on several CMS driven Technical Expert Panels as well as an expert resource for OASIS Updates. Her focus is on providing the knowledge and tools to operationalize external requirements while keeping the driver of care where it needs to be – the needs of each patient being cared for in the home setting. 

Cindy has been involved at the senior leadership level for the Home Health Section of the American Physical Therapy Association for more than 10 years and is the immediate past President of that organization. She has been working with APTA and CMS to clarify regulatory expectations and address proposed payment methodologies to ensure the long term participation of all services provided in the home health setting.

Originating Authors:

Iona Kaye (Katie) Barton, PT, Ed. D, MEPD. Katie received both her Bachelor’s of Science in Physical Therapy and her Master’s of Education in Professional Development from the University of Wisconsin – La Crosse. She received her Doctorate in Education Leadership from Saint Mary’s University in Minneapolis, MN. Katie was an adjunct instructor for the physical therapy program at the University of Wisconsin – La Crosse for 17 years. She has a varied background of healthcare experience including birth-3, hospital based pediatrics, school based PT, SNF PT, and most recently, 6+ years as a physical therapist for a hospital based home care and hospice agency.

Michelle Olson, M.S.P.T. Michelle received her Bachelor’s of Science in Biology from the University of Wisconsin – La Crosse and her Master’s of Science in Physical Therapy from Carroll College. She has been practicing as a physical therapist for a hospital based home care and hospice agency for the past 8 years and has had experience in sports medicine and pediatric home and clinic based care. Michelle has a special interest in physical therapist education and has also been employed as a lab assistant/associate lecturer at the University of Wisconsin – La Crosse for the past 5 years. Michelle is an APTA certified clinical instructor and has been taking students into the home care setting for the past 6 years.
