Fleming Date: August 2023JOB SUMMARYThe RN Case Manager, Bleeding Disorders has well-developed skills and knowledge in the assessment and care management of patients served by the Comprehensive Program for Bleeding Disorders (CPBD). The scope of practice includes patient/family assessment and management, resource management, utilization management, care facilitation, discharge planning, and other related duties specific to patients with bleeding disorders. The RN Case Manager collaborates with the Program Manager and Medical Directors of the CPBD to prioritize clinical issues and manage a caseload of pediatric and adult patients with varying degrees of health and psycho-social needs.The RN Case Manager, Bleeding Disorders will develop individualized care plans in collaboration with other resources and personnel, including but not limited to: primary care physicians and care team, Transitions of Care, Patient Resources, Complex Case Management, Emergency Departments, and Urgent Care to provide consistent goal-directed care. The RN Case Manager, Bleeding Disorders will provide comprehensive care, including chronic disease management and medication titration, within established protocols. He/she must be willing to travel to other clinical locations as needed.MAJOR RESPONSIBILITIESConducts Clinical Case Management/Clinical Practice and Quality of Care Assessments:Conducts a comprehensive patient and support system assessment at initial visit and at intervals necessary to update goals and plan of prehensively reviews patient health record, external health records, health status, and risk factors.Assesses the patient’s formal and informal support systems, including caregiver resources and involvement as well as available benefits and/or community resources.Interprets screening and selective laboratory/diagnostic tests. Maintains high level of skill and responsibility in all areas of ambulatory nursing, including but not limited to: symptom management, procedures (e.g. dressing changes, injections, etc.), patient education and communication, delegation protocols, medication management, documentation in the electronic medical record, and the management of urgent and emergent patient care situations. Develops critical knowledge of working with a specialty pharmacy and has the ability to be an expert resource in clotting factor medication along with blood products used for the treatment or prevention of bleeding. This RN has responsibilities for education/training with these specialty medications.Coordinates and facilitates patient care and progression through the care continuum for over 350 patients with a complex diagnosis (e.g. recognizing need for assisted living and working with physician and patient to support patient).Meets with patients and families to assess needs and develop an individualized care plan, collaborating with interdisciplinary team, including PCP and care team members outside UW Health.Initiates and/or facilitates referrals to other resources or agencies, e.g. skilled nursing facility, assisted care, Patient Resources. Communicates with primary care physician and members of the comprehensive care team regarding the status of the patient. Seeks consultation from appropriate disciplines/departments as needed and refers appropriate patients to other services, e.g. Dietician, Physical Therapy, Behavioral Health.Participates in clinical improvement activities. Assists with data collection and resource utilization for outcome measures, uses quality tools and processes to develop and support quality improvement projects, and facilitates data collection and documentation in the support and coordination of research.Provides educational offerings in area of expertise at UW Health, its affiliates, and in the community. Serves as a consultant to provide education regarding bleeding disorders and other therapies to patients, families, and healthcare practitioners within and outside UW Health and the community.
