Officer (OSO) feels has the unique qualities necessary to compete for a commission as a Second Lieutenant in the United States Marine Corps! The path to becoming an officer of Marines is long and arduous, and starts right here. Please take your time and fill out this application carefully, don’t leave anything blank and if you have any questions please call your OSO for assistance. As you progress down the path towards becoming a Marine Officer, and the going gets rough-and it will-remember the following quote and good luck!

The Platoon Leaders Class
Marine Corps Officer Training Programs
1. COMPLETION OF FORMS
a. Legibly type or PRINT all information.
b. Use your full name – as it appears on your birth certificate.
c. Complete all blank spaces with NONE or NA (not applicable) if it does not apply to you.
d. Pay particular attention to the Character Reference Section. Character reference letters will be mailed to those individuals from this office. Be sure a complete and accurate mailing address is provided. For “Other” references, DO NOT USE: relatives; girl/boyfriends; roommates or in-laws. Other military persons or professionals in other fields are wanted. If you are a Marine Reservist, be sure to provide the name of your Commanding Officer. You may also want to include your Platoon Commander or other fellow Marines. As a courtesy, ask individuals that you wish to use as references if they would be willing to complete a character reference prior to submitting their name.
2. REQUIRED DOCUMENTS TO BE FORWARDED WITH APPLICATION
a. Birth Certificate (a copy will be made and original returned to you)
b. Social Security Card (a copy will be made and original returned to you)
c. Current College Transcripts and all transcripts from past schools attended.
d. Verification of SAT/ACT/LSAT score.
e. DD Form 214 (prior service or reservist)
f. Copy of Service Record Book (prior service or reservist)
g. Copy of marriage certificate (if married), and children’s birth certificates
h. Provide you naturalization or citizenship papers (if not born in the US)
3. MEDICAL EXAMINATION
a. The Officer Selection Officer will schedule your medical examination.
b. If you wear any type of glasses, bring them with you.
c. If you wear contact lenses, DO NOT WEAR THEM for three (3) days prior to your physical examination.
4. FINAL INSTRUCTIONS
The Officer Selection Officer will assist you in pursuing your goal of becoming a Marine Corps officer. If you have questions regarding this application please call the Officer Selection Office. This is the initial step in preparing your application for the Marine Corps Officer Training Program. This office will assist you in the preparation and submittal of your completed application to the 6th Marine Corps District and ultimately to Marine Corps Recruiting Command for review. A selection board held at Marine Corps Recruiting Command, Quantico, VA, will determine final selection of your application. Keep in mind that this PROGRAM IS VERY COMPETITIVE. Keep pages 1 through 3 for your reference and provide the completed application by the date you and the OSO agree on. Page 2 outlines the normal timeline for submitting an application. It is crucial that you try to adhere to this schedule as closely as possible to ensure that application is completed in a timely manner.

This timeline is a best-case scenario and should be followed as closely as possible. The application process is extremely competitive. You will be competing with students from across the country for a limited number of openings at OCS. The sooner your application is completed and forwarded to the District Headquarters in Parris Island, SC for review, the better your chances are for selection. The national board takes place in Quantico, VA and consists of nine officers. Once the board has selected the “best qualified” applicants you will be notified by mail as to your selection or none-selection.
STATEMENT OF UNDERSTANDING

I understand that the Officer Selection Officer and the personnel of the Selection Office screen and evaluate applicants for the United States Marine Corps PLATOON LEADERS CLASS and OFFICER CANDIDATES COURSE commissioning programs. Final approval is the sole responsibility of the COMMANDANT OF THE MARINE CORPS. The only guarantees of this program are outlined in writing and are contained in Annex “B” and “C” of my contract.
I understand that I must take a “FULL MEDICAL PHYSICAL”; have at least a 2.0 cumulative last quarter/semester GPA; be a full-time student (12 semester hours); score at least 1000 on the SAT (45 combined Math and English on the ACT) or 115 on the “EL” portion of the ASVAB; meet minimum involvement standards with respect to legal authorities; meet minimum involvement standards with regards to illegal drug use; be less than 30 years of age when commissioned (27½ for pilots); take a “PHYSICAL FITNESS TEST” (see below), and be fully evaluated by the Officer Selection Officer before my application can be submitted. I understand that there are minimum requirements and that academic, mental, moral and physical disqualifications are not waiverable.
I understand that I must score a minimum of 225 points out of 300 on the Physical Fitness Test (PFT) and that it is scored as follows (Note: Males and Females have different scoring tables)

If qualified, my contract will guarantee pilot training with an eight (8) year jet and six (6) year prop/helo active duty obligation after I receive my aviator wings. The next three pages ask for information regarding any past illegal behavior on your part. Fill out each page completely. Police record checks will be run for every city in which you have lived, in addition to a national police check and security investigation prior to your commissioning. Any discrepancy between the following pages and the results of the national screening could result in your immediate expulsion from the Marine Corps and any commissioning program.
Insure to list ALL arrests, whether you were convicted or not, found innocent or not, released on your own recognizance, charges were dropped, records were sealed, etc. If a policeman detained you for any reason, list it. Juvenile records that are sealed are sealed to the public only. The federal government can and will access these records so list any police involvement.
List any types of recreational drugs (except alcohol), illegal drugs that you have experimented with, used, or tried. Keep in mind that during your security investigation people that you list as friends and acquaintances will be called to provide character references and asked if they have ever witnessed you using illegal drugs.
Finally, if required, ensure you provide a thorough statement detailing the where, what, when, and especially why, to any non-traffic arrests and/or drug use.
If there is more than six traffic violations, copy the BLANK form first then continue with the seventh (7th) violation on the second form.
If there is more than one Non-Traffic arrest, copy the BLANK form as necessary and list each arrest on a separate form.
If you have used more than one type of illegal drug, copy the BLANK form as necessary and list each type of drug used on a separate form.
100 word statement on why you wish to become a Marine Corps Officer. Use page 11 to write a short 100-word essay on why you want to become a Marine Corps Officer. Insure that you keep to the point and keep the statement to close to 100 words as possible. A board of officers that will have neither the time nor patience to read a verbose 500-word synopsis of your life will view your application. Spelling, grammar, and punctuation count. Focus on what has drawn you towards the Marine Corps and how you feel those qualities that you possess would be an asset to the Marine Corps. Feel free to use a separate piece of paper to work on your first or second drafts as necessary.
