Radiography Graduation Checklist

In order to graduate I understand that I must ….

← Complete all mandatory competencies and any elective competencies to total a minimum of 49.

← Make up all missed clinical hours before the end of each semester.

← Complete exam logs.

← Complete all clinical checklists (Portables, C-arm and Orientation).

← Return completed clinical workbooks to the school.

← Complete all courses with a minimum of a 77% / C.

← Meet with financial aid on or before the last day of class.

← Provide financial aid with references of three individuals’ with three different addresses and three different phone numbers

← Complete an exit interview with the placement department on or before the last day of class.

← Return my film badge to the school. (A missing film badge will result in a charge of $20 to the student)

Regarding the ARRT board exam, I understand that….

← Applications can be mailed up to 3 months before graduation. By sending my application in early, more choices will be available for a testing date.

← Any convictions (a plea of guilty or nolo contendere to a misdemeanor or felony, any withheld adjudication, suspended sentence or military court martial) must be reported to the ARRT. Misdemeanor speeding convictions are not required to be reported unless they are related to alcohol or drug use.

← Any concern about a previous conviction record that may affect my eligibility can be determined through the pre application process. The pre application can be done anytime before applying to take the national board exam and the fee is $75.00.

← It is entirely the responsibility of the applicant to provide the required documentation of all criminal proceedings, charges and convictions.

← The cost for taking the 2007 ARRT boards is $150.00 and $125.00 for re-examinations. This is not included in tuition costs.

← I must have two photo ID’s to take the exam

Regarding the MRTBE license, I understand that….

← It is my responsibility to obtain through the MRTBE, an Arizona state license after I have successfully passed the ARRT exam and before employment.

← There will be a waiting period of 7-14 business days after the MRTBE has received my completed application before they will issue an Arizona state license.

← The fee for the MRTBE is prorated to your birth month
