Accreditation Workshop Registration Form

The 2008 Standards Accreditation Workshop is scheduled for Saturday, September 8, 2012 from 8 AM to 5 PM at the Doubletree Paradise Valley Resort, Scottsdale, Arizona

Please follow these steps to register:

Please note: Seating is limited to 25 individuals; no on-site registration will be accepted.

You will be admitted in the order that the application and fee are received. We will send you an email that confirms the receipt of your registration form and fee for this workshop.

STEP 2: Submit the Registration Form. Submit the registration form to the Accreditation Department by Friday, August 6, 2012. E-mail the completed registration form to Irene Lau at ilau@.

The hotel room reservation information is on the next page. You must make your own hotel reservations. A block of rooms has been reserved until August 15, 2012.

STEP 3: Submit the Fee. The $200.00 workshop fee can be waived for one representative of currently accredited CAAHEP medical assisting programs who did not have a participant at the previous 2008 Standards Accreditation Workshop. All others should submit the $200 fee:
