Student Referral Guide

Students who you believe may need additional support or services may be referred to Student Affairs using a referral form. The Office of Student Affairs welcomes referrals from Academic Mentors, Chairs, and Faculty. The Office of Student Affairs staff, Dr. Laura Kiralla – Dean of Student Affairs, Michael Richards – Assistant Dean of Student Affairs, Julie Bryan – Assistant to the Dean of Student Affairs, Dr. Fred Barnes – Counseling Services, Dr. Carol Branch – Student Resources, Denise Gianoussopoulos - Career Services, all value input from the academic community on student well-being.

Each of the individuals listed above will interface with a slightly different focus in the case of a student issue. Please use your critical discretion in determining where to send a student. As a rule of thumb, any issues related to academic life—classes, faculty, degree requirements, grades, etc.—should be referred to the Department Chair. Issues that are personal, interpersonal, or emotional should be referred to the Office of Student Affairs.

We all know the difference between generic college adjustment issues and serious concerns that require referrals. Otis would rather that we act on the side of precaution, hence refer even when you are mildly concerned.

Listed below are some of the issues that you would refer. When a referral is necessary, it is important to inform the student that a referral will be made and to refrain from writing any highly personal information on the student interview forms filed in 506; give more detailed information on the referral form attached.

Steps for referring:

1. Fill out Referral Form available on the AMP web page—the date of referral is critical

2. Print two copies of the completed form

3. Place one copy in an envelope in Siri Kaur’s mailbox, Room Ahmn 506

4. Provide the second copy to the appropriate individual listed above (all Office of Student Affairs staff are located on the 1st floor of the Ahmn Building – Goldsmith Campus)

• If a student is in violation of the code of conduct (refer to student handbook), please complete the code of conduct complaint form at

(Otis Home page >Staff life>Forms>Student Affairs)

• If a situation occurs and you want to talk it through before you fill out a referral form, feel free to contact the Office of Student Affairs

• Often acting out or behavior problems are signs of deeper issues—pay close attention and refer when in doubt.
