Statement of Work

Janitorial Services

Scope

The contractor will furnish all labor, cleaning supplies and equipment to provide aseptic cleaning procedures at the VA Outpatient Clinic located at 104 Alex Lane, Charleston, WV 24304. Contractor will supply and maintain a MSDS for all chemicals or cleaning agents used at this facility. All chemicals or cleaning agents will be properly labeled at all times.

The contractor will be responsible for cleaning all areas listed below (Requirements Section) in the frequency noted for each area. Any changes to the location of areas to be cleaned or frequency of cleaning the areas will be coordinated with the COR (Contracting Officer Representative) and CO (Contracting Officer) of the contract prior to initiation of such change to assure adequate contractual coverage.

Breakdown of the Charleston CBOC:

➢ 5600 square feet of space

➢ 31 Rooms (Exam Rooms, Observation Rooms, Waiting Room, etc.), 3 rooms are carpeted which includes the Pharmacy, Mental Health Teleconference Room and Mental Health, all other rooms are tiled.

➢ 4 Bathrooms

Requirements

The awarded janitorial services company is required to perform all required duties between 1:30 PM and 7:30 PM Monday thru Friday. Contractor will be off duty on weekends and federal holidays. The contractor will develop facility cleaning procedures and shall ensure effective compliance with all provisions contained in the contract including infection control procedures (i.e. changing out mop heads on a regular basis). The awarded contractor will be required to take the Rules of Behavior (ROB) and Privacy training annually.

The following services will conducted in the time frame noted:

Performed Daily (Monday thru Friday)

• Empty wastebaskets and replace with clean liners. Spot clean and clean wastebaskets as necessary

• Dust all furniture including desks, chairs, tables and phones, etc.

• Dust all exposed filing cabinets, bookcases, shelves and surrounding areas to include windowsills, ledges, picture frames, etc.

• Clean and sanitize drinking fountains

• Clean reception lobby glass including front door, pharmacy windows and other partition door glass

• Dust and damp mop resilient and hard floors and/or vacuum carpeted floors in their entirety

• Spot clean furniture

• Pick up trash/garbage around building and sweep sidewalks, empty ashtrays

• Clean, sanitize and polish all vitreous fixtures including toilet bowls and hand basins

• Clean all glass and mirrors

• Empty all containers and dispensers and insert liners as required: spot clean and sanitize containers

• Spot clean all walls, doors and partitions

• Refill all dispensers to normal limits

• Sweep, damp mop and sanitize hard floors

Performed Weekly

• Remove dust and cobwebs from ceiling areas and vents

• Vacuum, clean furniture

• Spray and buff open areas including kneeholes of desk in general offices and reception area

• Clean and sanitize all trash containers

• High/Low dusting

Performed Quarterly

• Strip once per year, scrub three times per year; clean, refinish with 5 coats of finish and machine polish tile and shampoo carpet

• Clean all walls, doors, partitions and furniture

• Clean all windows

Facility Security

Contractor personnel shall not lend keys or open locked rooms or areas to permit entrance by persons other than the contractor and/or representative performing assigned duties. All rooms found locked are not to be left unattended while performing duties under this contract and shall be re-locked by Contractor and/or representative upon completion of duties.

The contractor and/or representative shall turn off all lights in unoccupied areas unless otherwise directed by the COR. When the contractor and/or representative open windows, they are also to close and lock the windows before leaving the area. If any difficulty is encountered in keeping areas locked, the COR shall be notified.

It is the responsibility of the Contractor and/or representative to ensure that all articles found by the contractor and/or representative are turned in to the COR.

Any keys given to the contractor and/or representative are the responsibility of the contractor and/or representative. If lost or stolen, the Contractor and/or representative must reimburse the facility for not only the key, but also any labor and materials involved in replacing cores and keys.

Safety and Fire Prevention

In the performance of this contract, the contractor and/or representative shall take such safety precautions as the COR may determine to be reasonably necessary to protect the lives and health of occupants of the building. The COR will notify the contractor and/or representative of any noncompliance with the foregoing provisions and the action to be taken.

The contractor and/or representative shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the contractor and/or representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the contractor and/or representative fails or refuses to comply, the CO may issue an order stopping all or any part of the work and hold the contractor in default of the contract.

The contractor and/or representative shall comply with applicable Federal, State and local safety and fire regulations and codes which are in effect at the beginning of the contract period, (i.e., OSHA, JCAHO, EPA). The contractor and/or representative shall keep abreast of and comply with changes in these regulations and codes applicable to the contract.

The contractor and/or representative shall follow applicable facility policies concerning fire/safety/disaster programs.

Contractor shall display appropriate warning signs when job operations may cause traffic obstruction of personnel hazard. Wet floor signs and other warning signs must be visible and placed so as to protect building occupants. Personal protection Equipment must be worn as necessary.

Conservation of Utilities

The contractor shall be directly responsible for instructing employees in utility conservation practices. The contractor and/or representative shall be responsible for operating under conditions, which preclude the waste of utilities, which shall include, but shall not necessarily be limited to:

➢ Lights shall be used only in areas where and at the time when work is actually being performed.

➢ The workers will not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems.

➢ Water faucets or valves shall be turned off after the required usage has been accomplished.

➢ Government telephones shall not be used for personal reasons nor any toll or long distance calls.

IT Security: The Certification and Accreditation requirements do not apply, therefore a Security Accreditation package is not required.
