Dental X-ray Internship Form. (form is 5 pages dentist only needs to sign page 1; the rest is your background info).

Provide the internship site with the following forms: 1. Copy of your resume (Perfected with A4DA listed under Education - See Section 3 online for EXACTLY what to add under Education) 2. Copy of Student Liability Insurance 3. Letter to Internship Site from School 4. EFDA 200 Hour Internship Form 5. Radiology Internship Form (Page 1 only) 6. Hours Log ? this is for your records to record your times and dates.

Finding an Internship Site To obtain an internship you will have to do some work. I personally suggest you go into multiple dental offices, ask to speak with the office manager, tell her you are a dental assistant student and would like to complete an internship with their office, if they have time to speak with you then you can provide the internship forms and explain the process or if they are busy ask to leave your resume and the letter from the school explaining the internship.

You may have to contact 10 different dental offices seeking an Internship. You can call or email a potential Internship Site but I have found it makes a bigger statement if you actually walk into the dental office and hand them the forms and explain your requirements. An email can be ignored, deleted or forgotten, but if you walk in the office looking PROFESSIONAL and drop off the forms, or ask to speak to office manager, you have a better chance of them taking the time to speak with you.

The days and hours of your internship are decided between you and the dental office but please remember it's 200 HOURS within 3-12 months. IT CANNOT BE DONE ANY QUICKER THAN 3 MONTHS and you MUST start your internship within 1 month of completing the 12 week course. It's an average of 2 days per week, to finish in 3 months. Please keep in mind although this is an "internship" it should be treated as if it is your job. You must be professional and show up to every scheduled day ready to work and learn. Be assertive, ask questions, take notes, and be friendly. You are not only representing Academy for Dental Assistants but are representing yourself in the hopes of being hired.

You can utilize the list of possible internship sites we have on our job/internship board in our facility or the same list is online on our website once you log in. Please keep in mind the list of Internship Sites is not a guaranteed of placement it is a list complied of dental offices that have contacted us looking for interns. At the time you are seeking your internship those offices may not have an internship available but you should try each office in your area to check if they would currently accept an intern.

Our school has Affiliation Agreements with Greenberg Dental and Orthodontics, Great Expressions Dental and Aspen Dental. You are not required to find an Internship site from our list, your Internship can be completed with any Florida Dentist.

We have also added an "Internship Sites " Tab that you can access from our website when you log into your account. We will add the available internships as they come in. You may want to look for an internship near your home that you may want to continue to work at after your internship. Many internships lead into paid positions.

After the Internship is Completed

Once you have completed the 3 month/200 hour internship the dentist must sign 2 internship forms: the EFDA 200-hour internship form and Dental Radiology Application form (page 1 only). Return those 2 forms to Academy for Dental Assistants once you have completed your 3 month/200 hour internship and we will issue your 2 Florida Board of Dentistry Certifications (EFDA and Dental Radiology Certification).

The Hours log is for you to keep track of your 200 hours. I suggest providing a copy of all the internship forms to the Internship Site during the interview so they are aware of what is needed. It is your responsibility to keep track of the forms and your hours throughout the internship.

Keep in Mind:

? Your internship must be done in a General Dental Office or Pediatric Dental Office, not a specialty office like Oral Surgery, Endodontics, Periodontics (this is because specialty offices only do specific procedures and not enough EFDA procedures)

? You will interview for the internship just as if it where a job, dress professionally (clean, pressed scrubs are OK to wear to a dental interview) and be prepared for interview questions.

? While at your internship - You need to be assertive - ask questions - jump in to help the assistant and doctor - if you feel you aren't getting a learning experience talk to the doctor, assistant or office manager about what you're needing to learn.

? The Dental field is a close community always be professional, respectful, and polite. ? DO NOT EVER HAVE YOUR CELL PHONES IN YOUR POCKET OR TEXT WHILE AT YOUR

INTERNSHIP - (Biggest complaint I get from the dentists) ? When your Internship is completed, send our Office by email, fax or drop off the 2 forms that the dentist

has signed. Once we received those forms you will be issued your 2 Florida Board of Dentistry Certifications (EFDA and Dental X-ray Certification from the Board of Dentistry) ? Once you have completed your internship you should add your internship site to your resume under "Experience" with the name of the office and bullets of all the procedures you did (JUST LIKE IT WAS A JOB) because now you do have some dental experience.

If you have any questions please feel free to speak with your instructors and they can answer any questions.
