Neurological Disorders (Parkinson’s, Huntington’s Muscular Dystrophy, MS, Myasthenia Gravis)

Progression of Disease within last 12 months

1. Progression from independent ambulation to wheelchair or bed-bound status

2. Progression from normal to barely intelligible or unintelligible speech

3. Progression from normal to pureed diet

4. Progression from independence in all or most ADL’s to requiring assistance in all ADL’s

One or more of the following must also apply:

1. Critically impaired ventilatory capacity, evidenced by:

- Vital capacity (VC) less than 30 % of predicted

- Significant dyspnea at rest

- Supplemental oxygen required at rest

2. Critical nutritional impairment evidenced by:

- Artificial feeding not elected or discontinued – oral intake insufficient to sustain life

- Continued weight loss

- Dehydration or hypovolemia

1. Life threatening complications evidenced by:

- Recurrent aspiration pneumonia

- Decubitus ulcers, multiple, stage 3-4, particularly if infected

- Recurrent Urinary tract infection

- Sepsis

- Fever recurrent after antibiotics
