Welcome/Introductions: Ginny Donney of Shawnee Health Service welcomed everyone to the meeting and introductions were given of all in attendance.

Approval of Agenda: Agenda stands approved.

Approval of March 22, 2012, Meeting Minutes: Minutes stand approved as written.

Agency Presentation: Colleen Flanagan of TASC, Inc. gave this month’s agency presentation.

TASC (Treatment Alternatives for Safe Communities) is a statewide agency that provides opportunity for individual change through advocacy, direct service, and system enhancement.

TASC advocates for people in courts, jails, prisons, and child welfare systems who need treatment for alcohol/drug and mental health problems. Clients who receive TASC adult court and probation case management services are twice as successful in treatment. TASC reaches approximately 20,000 individuals each year throughout Illinois, and offers training and consultation services nationally and internationally. Thousands of people with substance abuse and/or mental health problems are mandated to TASC by criminal justice, corrections, and child welfare authorities. Results show that TASC clients are more successful, in terms of reduced drug use and crime, than their counterparts who don’t go through TASC.

TASC works with state administrative bodies, public systems, and community-based agencies across Illinois to deliver comprehensive recovery management services to individuals with substance abuse and mental health disorders. TASC’s services also address related issues such as criminal and delinquent behavior, under-employment, housing, and under-education.

TASC’s services are guided by core clinical values that support positive behavior change.

TASC:

▪ Provides services that help individuals use their own strengths to find solutions to life problems

▪ Recognizes that addiction is a chronic illness that can be treated, and that recovery can be managed

▪ Motivates and engages clients in the recovery process

▪ Respects all individuals with whom TASC works

▪ Focuses on successful outcomes with a recognition that lasting behavior change is incremental and takes time

Action Plan Updates

Miriam Link-Mullison gave a brief overview of the structure of the Jackson County Healthy Communities Coalition, Steering Committee, and Action Teams.

- Jackson County Healthy Communities Coalition is a group of individuals that represent their agency/organization to better the community by addressing priority health needs, share information, and collaborate on projects.

- JCHCC Steering Committee keeps track of the overall healthy community coalition and organizes the agency presentations, etc.

- Action Teams focus on the individual priority health needs and this is where the bulk of the work is being done. Everyone involved in the JCHCC is encouraged to join an action team to address specific issues.

▪ Access to Care Action Team

• Access to Care is a Joint Access Team which includes Jackson, Franklin and Williamson Counties.

• Focus continues on increasing access to oral health services in Jackson, Franklin, and Williamson Counties.

• Miriam Link-Mullison reported that the oral health project has had a huge impact on emergency room usage. Data shows in the first complete calendar year, 2011, of the pilot program SIH referred 131 patients for emergency dental appointments. Almost 50% of those referred (63) kept their appointment and received a resolution for their oral health concern.

• The Access to Care Action Team will start looking at mental health issues.

• The Web-based E-Referral Manual for Jackson County and the surrounding region has been updated and is located on the SIH Website and on the Community Health Calendar. This web-based referral manual has information regarding primary care, dental care, mental health services, financial support, housing assistance, wellness and linkage resources, etc. Most resources link directly to an agency’s website so you can be sure you are receiving the most up-to-date information. ▪ Behavioral Health Action Team

• The primary focus of this action team is to continue submitting articles for the “Living Well” column for the Southern Illinoisan newspaper which comes out in the Mind & Body section of the Southern Illinoisan each Tuesday. The Behavioral Health Action Team is constantly looking for ideas for these articles. Contact Karen Freitag or Jean Alstat at (618) 457-6703 or Colleen Flanagan at (618) 565-0060 with suggestions.

• Next meeting: June 5, 2012 @ 8:30 a.m. at Southern Illinois Regional Social Services. (This group meets the first Tuesday of every month at SIRSS (Turning Point Building).

▪ Sexual Health Action Team

• The new co-chair for the Sexual Health Action Team is Jason Gillman.

• The overall goal of the Sexual Health Action Team is to reduce STDs in Jackson County. The specific strategies chosen by the team include: Working with providers to increase the rates of screening for STDs in Jackson county, working with educators and community agencies to increase the numbers of young adults who receive education that includes abstinence, prevention of STDs and birth control methods and increase the number of young adults who use barrier protection against STDs.

• The Sexual Health Action Team will be conducting sessions at the upcoming Women’s Health Conference in September and also in the process of working with churches to address these issues.

• Working on increasing screenings for Chlamydia. Recent studies show that rates are up so will work on prevention measures with youth, parents, schools and physicians.

• Always looking for new team members. Would like to recruit parents who are interested and supportive of comprehensive sex education in schools. Please send an e-mail to Miriam Link-Mullison at mlm@ if you would like to join this action team.

▪ Healthy Living Action Team

• The overall goal of the Healthy Living Action Team is to decrease cardiovascular disease including diabetes and obesity issues and to increase awareness regarding nutrition, physical activity and tobacco usage.

• Project Power – This faith based initiative continues and is in nine African American churches in Carbondale and in the process of adding Bethel AME Church and looking for one additional African/American church. The program teaches individuals how to make healthy choices to increase their overall health. Powers to end Diabetes program was held on May 19, 2012 at the Boys & Girls Club of Carbondale with approximately 35-40 in attendance with 27 being screened. Work continues to keep all churches engaged. The Pin a Sister event will be held on Sunday, May 27, 2012 at 4 p.m. at Rock Hill Missionary Baptist Church.

• Diabetes Today Resource Team – The main goal of this committee is to educate healthcare providers. A Diabetes Resource Guide was developed and can be located on the SIH Website as well as the Community Health Calendar. In the process of updating the resource guide. Changes are due by June 7, 2012. Outreach to health care providers will be held on June 4, 2012 at Center for Medical Arts. Contact Angie Bailey at (618) 684-3143 if interested in joining this resource team. The next Diabetes Today Resource Team meeting will be held on July 12, 2012 at 11:00 a.m.

• The Healthy Living Guide is located on the SIH website () under member tools and it is also located on the Community Health Calendar.

• Community Health Calendar – All were reminded to use the online Community Health Calendar. All items listed on the health calendar must have an educational component and be open to the public. (no fundraisers). The community health calendar can be located at: munityhealthcalendar.siuc.edu. New business cards are now available to promote the calendar. The Community Health Calendar also has a variety of tools and links for the Healthy Living Guide, E-Resource Manual, Diabetes Guide, submission forms for START Walking paths, etc.

• Community Leaders Forum for Franklin/Williamson counties is scheduled for October 25, 2012 at 8:30 – 11:30 a.m. at John A. Logan College. This will be an invite only event for community leaders.

• Farmer’s Market Update – Orlan Mays continues to work with area churches on community gardens and will start working with the City of Carbondale as well.

• CATCH – Coordinated Approach To Child Health

➢ CATCH has been implemented in 58 out of 102 schools within the Delta Region and continues to grow. The goal is to have all 102 schools on board by 2015.

➢ Plans continue with the construction of a START! Walking path at the Superblock and Carbondale Community High School.

➢ Jump with Jill program came to 12 schools recently in southern Illinois.

➢ Working with the Boys & Girls Club of Carbondale on after school programs.

➢ Bike Fest held on May 19, 2012 at the Boys & Girls Club of Carbondale.

• START! Walking paths – Over 50 paths have been created so far in southern Illinois. For a listing/map of all locations, Google START Walking paths in southern Illinois. These paths are also located on the SIH website as well as the American Heart Association’s website. If you know of a location to create a START Walking path. On-line submission forms for walking paths are available on the Community Health Calendar.

• Work continues to reduce CVD (including diabetes & obesity and tobacco use) in Jackson County. Working with the Healthy SI Delta Network on a coordinated approach with 16 counties in southern Illinois.

• Healthy Weight Collaborative

➢ Childhood overweight/obesity toolkit has been completed. These toolkits are for physicians to use in their practices to support childhood obesity/prevention. The toolkit also includes parent resource information.

➢ Met with SIU Family Medicine to provide these toolkits. Will also be meeting with Center for Medical Arts on June 4, 2012. This is a multi-agency/team effort.

• Tobacco Toolkit for providers will be forthcoming.

• Community Transformation Grant – “We Choose Health” is a multi-year effort to encourage and support population-based obesity and tobacco prevention in Illinois communities. The Jackson County Health Department in partnership with the Healthy Southern Illinois Delta Network (HSIDN) in the lower 16 counties of Illinois will be applying for this grant. The HSIDN will be writing two separate applications for similar programming. Our partnering communities of Egyptian Health Department, Franklin-Williamson Bi-County Health Department, and Hamilton County Health Department will also be applying through a separate application. All work will be conducted through established Healthy Communities Coalitions. With this grant application we will address healthy eating, active living and smoke-free living. ▪ Traffic Safety Action Team

• Concentrating on motorcycle safety since May is “Motorcycle Safety Awareness Month”. Had a presence at the Blessing of the Bikes event in April

• Working with the Designated Driver Program on June 16, 2012

• In the process of working with Kroger on the BigK/Coach Kill Cancer Fund Poker Run scheduled for August 25, 2012.

▪ Save the Date: 2nd Annual Disaster Conference “Weathering the Storm” – October 16, 2012 at 8 a.m. – 4:30 p.m. at Rend Lake Resort.

▪ Polar Bear Event Update – Jason Gillman of SIU Student Health Center gave an update on the Polar Bear Event held in January. Jason reported that this year’s event went much better than in the past. A town hall meeting on underage drinking was held on April 3, 2012 at the Student Health Center Auditorium. The event and was well attended and was very good. The task force for this event is working on an action plan for the future.

▪ Chronic Disease Management Workshop – Free 6 week workshop on Chronic Disease self management for adults or their care takers. You will get support and tools to deal with pain and fatigue, improve nutrition and exercise choices. Come learn to take charge of your health, to live well and be well while coping with diabetes, arthritis, high blood pressure, heart disease, chronic pain or anxiety. Participants must pre register call 618-519-9487. Class runs for 6 weeks June 14 through July 19, 2012 on Thursdays from 2- 4:30 p.m. at Shawnee Health Care Carbondale. ▪ Rosemary Berkel Crisp Award – Nominations are sought for the Rosemary Berkel Crisp Award. Women for Health and Wellness, Inc. is seeking a woman who is committed to healthy families and healthy communities; a woman who has contributed to the advancement or support of others or her community through her volunteer efforts or personal contributions. ▪ Pin-A-Sister Sunday – May 27, 2012 @ 4 p.m. at Rock Hill Missionary Baptist Church. The pinning ceremony is to raise awareness about the importance of breast cancer screenings for African-American and Hispanic churches across Illinois and several states throughout the nation. ▪ Migrant Farm Workers – Karla Shetter, Migrant Program Coordinator for Shawnee Health Service informed the committee that there are migrant workers that are new to the area that will be here from May through October. As the Migrant Outreach Coordinator, Ms. Shetter conducts outreach, health education, and case management services to farm workers and their families.
