Safety Huddle Template



• It is the responsibility of the charge nurse to initiate a safety “huddle” at the beginning of every shift to pass on relevant safety information about patients, families and the work environment.

• The information should be shared between charge nurses at beginning/end of the shift.

• The huddle should be short (2-5 minutes). The goal is to collect and share information about potential safety issues and concerns on a daily basis. All RNs/LPNs/CCAs on the unit should be aware of the issues that need to be addressed with the patients.

✓ On the last shift were there safety issues related to medications/ IV solutions, any other near miss events with equipment, medications, infection control practices, etc? What needs to be done to address these issues? (It is the responsibility of the provider involved in an issue to fill out a PSRS report or SAFE report).
