Service Description

Care Coordination will be managed at two locations by a Senior Care Coordinator and incorporated into Gaudenzia's treatment continuum. The team of coordinators will be collectively responsible for transitioning clients to the next level of care (if appropriate) by removing barriers, providing reminders, conducting outreach and developing and maintaining relationships with services and programs. These linkages will include but not be limited to treatment, family counseling, health care (including assistance and applying for PAC and Medicaid when applicable), employment, legal assistance, housing, child welfare, social services, transportation and peer-support providers. The Care Coordinators will interface with clinical staff for review and verification of recommended services, identification of health care and recovery support needs, initiating referrals to the clinically recommended level of care and maintaining contact information. They can assist clients with scheduling appointments, arranging transportation and understanding any instructions they may receive. The Care Coordinators will work as part of the treatment team which establishes a Recovery Plan in coordination with patient and primary clinical counselors. The Care Coordinator will continue to provide these services post discharge for a period of 6 months.

Service Description

HealthCare Access Maryland (HCAM) provides care coordination and case management services for clients enrolled in the ATR grant who are returning from treatment to Baltimore City. HCAM will meet with clients at their inpatient facility (or via phone if necessary) prior to their release from treatment to discuss care coordination services. Follow-up meetings will be held at treatment facilities, patient homes/transitional houses/recovery houses, at the HCAM offices or other facilities as agreed upon by the Care Coordinator and client.

A Step Forward, Inc. is a not-for-profit alcohol and drug (substance abuse) treatment program established in Baltimore, Maryland in 2001. Service Description

Gaudenzia provides comprehensive community-based substance abuse treatment, mental health treatment and primary medical care services. Gaudenzia strives to provide professional, flexible, responsive treatment to meet the treatment needs of each client. Gaudenzia's staff is client-centered and an emphasis is placed on empowering clients. Gaudenzia used a modified therapeutic community modality and has been a treatment provider for over 40 years.

Gaudenzia offers an extensive range of services including detoxification, intermediate care, halfway house services, intensive outpatient and outpatient addictions treatment, long-term residential services and coordination of psychiatric services, individual therapy and a dual recovery program.

Gaudenzia offers a structured halfway house program in a new building which houses our long-term residential treatment program for males.
