HELOC DEBIT CARD APPLICATION

IMPORTANT NOTES: This application is for an access device called a "HELOC Debit Card" that can be used to draw from and access your home equity line of credit (HELOC) with Leader Bank. This card is not associated with any deposit or asset account, and is connected solely to your Leader Bank HELOC.

CHANGES IN TERMS: This card is a credit access device for your HELOC, and unless expressly set forth below, all terms and conditions of your home equity line of credit as set forth in your Standard Revolving Credit Agreement and Note, Important Terms of Our Home Equity Line of Credit disclosure, Revolving Credit Mortgage, and all other disclosures given to you at or before the consummation of your home equity line of credit shall remain unaffected by your request for a HELOC Debit Card. The changes to your home equity line of credit terms as a result of your request of the HELOC debit card are as follows, and by executing the HELOC Debit Card Application, you affirmatively agree to these changes as a condition of receiving a HELOC Debit Card:

Minimum Draw Requirement: The minimum initial draw that you can receive is $25,000.00, unless otherwise authorized by us. However, you need not take the initial draw at the time your line of credit closes. For credit advances obtained through any HELOC debit card issued to you, there shall be no such minimums applicable to such credit advances.

Additional Credit Draw Restrictions: Any credit draws received by use of a HELOC debit card must be received through point of sale transactions; automated teller machine or PIN-based transactions are not authorized through such cards. Credit draws received through use of your HELOC debit card are limited to $2,500.00 in total per calendar day.

You understand that by signing this application, you agree to the changes to the terms of your Home Equity Line of Credit as set forth in the "Changes in Terms" section set forth above, which shall become effective upon the issuance to you of the card. Your debit card will not be assigned a PIN and therefore cannot be used to make ATM withdrawals and/or used with merchants who only accept debit PIN transactions. You also understand that as part of receiving a HELOC debit card, you will be automatically enrolled to receive eAlerts that will be delivered via email for every transaction completed with your debit card. Due to an increase in fraud originating from within and outside of the USA, your debit card may not be available for use in some states or for e-purchases, and no transaction with your debit card will be authorized in foreign countries. As fraudulent activity is detected, areas may be added or removed from this blocked list as necessary.
