Longitudinal Outpatient Experience (LOE) GuideFormerly called Continuity Clinic2018-2019University of Florida College of Medicine – JacksonvillePediatric Residency ProgramResident ResponsibilitiesPlease be on time for your Morning and Afternoon clinics. Morning clinics start at 8:00 and afternoon clinics start at 1:30, unless informed otherwise by your preceptor. Keep your Morning and Afternoon Days of the week the same throughout the academic year. No switching of days is allowed unless Dr. Hipp or the Pediatric Chief Residents are notified. In order to establish longitudinal care, your preceptor and your patients need to know when you are going to be there. Please notify your preceptors of any dates that you will not be in clinic including vacation dates, post-call dates, and night shift/ night float months. Provide your preceptor a copy of your Block Schedule for the academic year. Please give them as much advance notice as possible. This is your responsibility. The ACGME requires you attend 36 sessions of continuity clinic per year. The Residency Review Committee (RRC) suggests that:PL-1 see a minimum of 3 patients per sessionPL-2 see a minimum of 4 patients per sessionPL-3 see a minimum of 5 patients per sessionBe a true coordinator of care for your patients. Remember to follow-up on patient visits, labs, imaging, and referrals.Keep a log of the number of patients seen per clinic day on New Innovations in the Log Books section.Provide your preceptor and the residency program with feedback in order to improve your experience. Evaluations are completed twice a year on New Innovations. The process of evaluation and feedback should be continual and bidirectional throughout the academic year.Enjoy the patients and your clinic experience!INTERN ASSIGNMENTS 2018-2019InternLocationPreceptorDay Rachel AzumbradoUF – San Jose6271 St. Augustine RoadJacksonville, FL 32217Dr. Carmelo CedresTuesday AMWednesday PM Esther Bell UF- Monument Landing2485 Monument LandingSuite 12Jacksonville, FL 32225 Jacksonville, FL Dr. Sylkia MartinezThursday AMMonday PM Alexia EigesUF – San Jose6271 St. Augustine RoadJacksonville, FL 32217 Jacksonville, FLDr. Pamela LindorWednesday AMThursday PMIssa HannaAgape- Blanding5460 Blanding Blvd. Jacksonville, FL 32210Dr. Mikah OwenTuesday AMThursday PM Bayan IssaUF – E.
