Notes:

II. Only authorized purchasers included in the State of Washington Purchasing Cooperative (WSPC) and State of Oregon listings published and updated periodically may purchase from this contract. It is the contractor’s responsibility to verify membership of these organizations prior to processing orders received under this contract. A list of Washington members is available on the Internet , and a list of Oregon members is available on the Internet . Contractors shall not process state contract orders from unauthorized users.

III. Contract Terms: This Document includes by reference all terms and conditions published in the original IFB, including Standard Terms and Conditions, and Definitions, included in the Competitive Procurement Standards published by OSP (as Amended).

Special Conditions:

1. 07/20/09: This CCI serves as expiration notice; contract will expire October 4, 2009 without being replaced. Until the expiration date all terms, conditions, specifications and pricing remain unchanged.

2. 08/06/08: Update OSP alternate contact information.

3. 04/30/08: With this CCI contract is being extended until October 4, 2009. All other terms, conditions, specifications and pricing remain unchanged

4. 04/01/08: Update contractor contact information.

5. 12/31/07: This CCI is being issued to update OSP contact information.

6. 10/05/04: This is a new contract developed at the request of the Military Department and will be utilized to implement the federally funded “Homeland Security” project within the State of Washington.

7. Emergency Purchases: supplier agrees, to the extent possible, to honor requests for “emergency purchases”. Any additional costs incurred for special delivery will be borne by the purchaser. In the event supplier is unable to meet such requests, purchaser(s) will be authorized to purchase their immediate requirements from non-contract suppliers.

SPECIFICATIONS & PRICING

Decontamination kit, individual, personal identity. Provides secure storage, identification and traceability for a victim and their belongings during/after the decontamination process. Three separate bags, Bag A is for victim’s valuables, Bag B is for contaminated clothing and Bag C contains gown, etc.

• Bi-lingual, both English and Spanish instructions combined with pictures to make use easy to understand

• Barcoded identity bracelet, three snap, with personal effects allows traceability of personal belongings by first responders. Barcodes with man-readable numbers link the bracelet to both personal affects bags

• One size fits all blue modesty redress gown, poncho style, with sewn sides provides privacy to wearer

• Re-sealable opaque “Valuables” bag

• 18” x 32” re-sealable clothing bag

• two large microbial washcloths (12” x 18”)

• Pair of one size fits all slippers with ribbed sole

• Three disposable towels, size 24” x 36”

• One large mylar blanket

Basic Instructions:

1. victim deposits personal valuables in first bag (Bag A) which is bar coded to a bracelet worn by the victim

2. once inside the “undress” stall, victim removes all contaminated clothing and places into second bag (Bag B) which is handed to the first responder

3. the third bag (Bag C) is taken into the shower unit then the “redress” area by the victim. After showering, the victim dries off with the three towels, then redresses using the clean gown and slippers, and is provided a mylar blanket for comfort

Bag A dimensions are 14” x 10.5” x 1” , is for valuables and is resealable opaque bar coded. Contains two microbial washcloths

Bag B dimensions are 18” x 32”, is for contaminated clothing and is resealable bar coded ziplock type

Bag C dimensions are 14” x 10.5” x 1” and contains the 3 pc snap lock bar coded identification wrist band, three disposable towels, smock, slippers and mylar blanket
