Notes to Contract Manager:

• If the performance security is in the form of a performance bond, you must first provide notice to the bonding agency of any material change such as increasing or decreasing the contract price, changes in completion time, location, nature of the contract works/services, etc., etc. Also, a copy of the fully executed modification agreement must be forwarded to the Bonding Agency.

• If the signature page creates page 2, ensure at least two lines of text are carried over as shown.

• Section “B” below is used to capture modifications such as adding a term to an option-to-renew contract or a change to a contract section, clause, or contract schedule requirement (e.g., time schedules, completion dates, raise or lower maximum fees and/or expense limits, performance standards or delivery requirements, etc.), but not an increase or decrese in the originally specified work (see Section C).

If utilizing section B only, delete section C.

• Section “C” below is used to capture modifications such as an increase or decrease to the originally specified amount of work (e.g., trees planted, maps produced, km of road built, etc.), add or delete work areas (e.g., ha to be traversed or mapped or spaced), or to substitute specified materials or products (more commonly for field operations or works contracts).

Unless otherwise indicated, lump sum or unit prices shown below constitute full payment for all labour, materials, equipment, overhead, profit, taxes (including taxes paid or payable by you to a supplier) and other things required to completely incorporate the change(s) into the contract work, but exclusive of Goods and Services Tax (GST) and any applicable Provincial Sales Tax (PST) that you are required to charge the Province as a taxable transaction.
