CHAPERONE INFORMATION
We are currently in need of approximately 40 chaperones for our trip to the National American History Museum on October 18, 2018. The number may vary due to student interest. If this happens, we will be holding a lottery in choosing our chaperones. We would really appreciate it if you would be able to chaperone our students on this trip. The cost for chaperones to attend is $30.
If you are interested in returning please complete the bottom of this form and return it no later than Friday, September 21st. Please do not send any money with this form. If you are chosen to be a chaperone, we will notify you, and money would be due at that time.
All volunteers must complete the volunteer application and participate in an orientation/training session annually. Directions can be found at:
There is a Volunteer Training Session at Parkville Middle School on September 20th at 6:00pm
