STATEMENT OF WORK, PHILIPS, 518-18-1-7441-0002SCOPE OF WORK: Service Contract for the Xcelera system in the Echo department located at the Bedford Campus and one workstation located at the West Roxbury Campus. Philips Xcelera, a Cardiology picture archiving and communication system (PACS), is an integrated, multi-modality image and information system designed to perform the necessary functions required for importing, exporting, storing, archiving, analyzing, quantifying, reporting and database management of digital medical Cardiology images.. The vendor shall provide all parts and labor to maintain and repair this system . All equipment shall be maintained in proper operating condition as specified by the manufacturer. Vendor must have proven access to original equipment, manufacturers (OEM) parts manuals and schematics to perform the service. Vendors must provide their own EOM manuals and schematics, which must be available on site at time of service. All defective parts replaced become the property of the vendor according to OEM.1. EQUIPMENT:The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work for the Xcelera Equipment at the VA Bedford Medical Center and one Xcelera Advanced Workstation at the VA Medical Center West Roxbury Campus This shall include, but is not limited to, all Personal Protective Equipment (PPE) and proper safety which may be required for the work to be completed in a safe manner.Vendor will provide technical telephone remote support and on-site support if maintenance cannot be resolved with remote technical or helpdesk support.2. AREAS TO BE SERVICED Bedford VA Edith Nourse Memorial Hospital 200 Springs Road Bedford,MA 01730 and West Roxbury VA at 1400 VFWY Parkway West Roxbury 02132 3. TASK FREQUENCY AND INSTRUCTIONS:This shall include the vendor to service and repair the the system be it a single day or multiple days, at the VA Bedford Echo department or VA West Roxbury Echo department, sufficient to complete the work set forth in the scope of work. This visit is to be scheduled in advance with the POC: Ryan Dragon of Clinical Engineering at 781-687-2718.Hours of Coverage: Contract Hours will be remote software technical support 24 x 7, Remote clinical application support M-F 8 x 5 (local time), Philips Remote Services (PRS), OnSite software escalation M-F 8 x 5 (local time) next business day, and Philips-purchased hardware support M-F 8 x 5 (local time) next business day are also includedResponse Time: Contractor shall provide phone call back by a qualified technician within 4 hour of service request and on-site response by a qualified technician within 48 hours of original request for service or at a later time specified by Biomedical Engineering. Contractor will report to Biomedical Engineering upon arrivalQualification of Vendor: Vendor will provide if requested, documentation of substantial experience working on the identified make and model instrument. Contracting Officer reserves the right to request factory training certificates from vendor personnel for servicing equipment under contract to determine qualification of responsibility.Specifications of Work: Contractor will follow manufacturer’s recommendations for repair and modifications, including use of diagnostic software where applicable. Replacement software and upgrades will include all site-specific adaptation present in originally delivered software. Guaranteed Uptime: A guaranteed uptime of 95% shall be maintained during hours of coverage to date: Uptime % = productive time – down time divided by productive time times 100. Productive time is planned time of use of equipment. Downtime begins at time of first service call until equipment is returned to proper operating conditions. Updates: Vendor will provide all software and hardware updates upon release at no additional cost to the Government throughout the life of this contract.Documentation Requirements: Service field reports shall be provided at the time of the service call.Services and parts not covered by contract: Contracting Officer’s authorization for any work or parts outside the scope of this contract must be received prior to performance of work or payment will not be authorized. Parts required that are not covered by contract will be purchased by the VA and installed under the terms of the contract4. SPECIAL WORK REQUIREMENTS: NonePROPERTY DAMAGE:The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, cubs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer.6. ATTACHMENTS:None.7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court.8. COMPLIANCE WITH OSHA AND ANSI LASER SAFETY STANDARDS:The contractor shall comply with the Federal OSHA and ANSI Laser Safety Standards.The contractor shall:A. Have methods by which all employees are educated as to risks associated with laser safety.B. Have policies and procedures which reduce the risk of employee exposure to lasers.C. Have mechanisms for employee counseling and treatment following exposure to lasers and laser generated airborne contaminants.D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract. ................
