APPOINTMENT

Sheffield Teaching Hospitals NHS Foundation Trust is seeking to appoint a Consultant in Oral & Maxillofacial Surgery with an interest in Head & Neck Oncology. This is a replacement post based at the Royal Hallamshire Hospital in Sheffield, with clinical sessions in the Charles Clifford Dental Hospital and at Mexborough Montague Hospital, Mexborough.

Any candidate who is unable for personal reasons to work full-time will be eligible to be considered for the post, in which case modification of the job content will be discussed on a personal basis with the Trust and in consultation with consultant colleagues.

School of Clinical Dentistry

The School of Clinical Dentistry at Sheffield ranks as one of the top Schools in the UK.  In the RAE 2008, 70% of our work was graded as world leading or internationally excellent and we were ranked fourth among UK Dental Schools.  The School achieved an “excellent” (23/24) rating in the last (2001) QAA Subject Review exercise, and in a recent independent review of teaching was described as ‘an excellent school of international calibre’. The School is consistently top in the National Student Satisfaction Surveys and in the good university guides.

The School has an intake of 79 dental undergraduates for its five year BDS programme. It has an annual intake of 30 students on the Diploma in Dental Hygiene and Dental Therapy course, which runs for twenty-seven months. In addition, the School has a significant annual intake of postgraduate students on taught courses and reading for higher degrees by research.

The School of Clinical Dentistry and the Charles Clifford Dental Hospital have recently completed a £14.5 million phased redevelopment. The first phase, a new £5.5m wing that includes new state of the art research facilities, seminar room space and office accommodation was opened in 2008. The second phase involved the redevelopment of the Charles Clifford Dental Hospital, which is adjacent and connected to the School. This phase, which was opened in 2009 included the upgrading of many areas around the hospital including new clinical facilities.

The University’s Medical School, the Health Sciences Library and the Royal Hallamshire Hospital are nearby and all are on the perimeter of the main University campus.

Our research is organised into interdisciplinary thematic groups reflecting our philosophy that progress is derived most readily from a multidisciplinary approach and that research should not be constrained by the traditional boundaries of clinical disciplines. We focus our research activity into areas of international excellence. We have four research groups Biomaterials, Dental Public Health, Oral Disease and Oral Neuroscience, each of which encompasses laboratory and clinical projects, addressing basic and applied questions. General Research Strategy

Our overall strategy for research is:

• to build on established strengths as identified by the four research groups,

• to develop areas of research that are responsive to national and international priorities, and

• to develop collaborations with other internationally leading scientists.

Research strategy is developed by the Dental School Research Committee which includes the four research group leaders and is chaired by the Director of Research, who also provides input to, and feedback from, the Faculty and University Research Committees. The Postgraduate Research Tutor and the School Dean also sit on the committee and there is representation from the Sheffield Teaching Hospitals NHS Foundation Trust (STH) to ensure a co-ordinated research strategy for the Dental School and Hospital. The Research Committee is responsible for overall research strategy, for allocation of internal research funds, for overseeing selection and monitoring of postgraduate students and for disseminating information to staff.

Some distinctive components of the Sheffield BDS programme include integrated teaching and learning of the clinical and scientific basis of dentistry based on interdisciplinary topics, a joint oral medicine, oral surgery/oral and maxillofacial pathology course; experience in the Dental Practice Unit (DPU) in the 4th/5th Year and electives. In addition we have an innovative outreach programme which involves primary care placements in General Dental Practices and Salaried Dental Services, which students attend for 20 weeks during the 4th and 5th years.

Some distinctive components of the Hygiene and Therapy Programme include substantial integration with the dental students. Hygiene and Therapy students learn clinical skills in restorative dentistry in mixed groups with dental students. In the second year, Hygiene and Therapy students also spend two days per week for approximately six months working in ‘Outreach’ where they will work in a dental practice or salaried service providing treatment in primary care.

DONCASTER AND BASSETLAW HOSPITALS NHS FOUNDATION TRUST

Doncaster and Bassetlaw Hospitals NHS Foundation Trust is a highly successful organisation, being one of the first ten Trusts in the country to be awarded Foundation Trust status and one of the few to have achieved Three Stars each year since the introduction of the Government’s “star ratings” system. The Trust has featured consistently in the list of the Top 40 Hospitals and was awarded a Government Charter Mark on three consecutive occasions. The Trust was a ‘pilot site’ for the 18 week pathway and in December 2007, was one of only three Trusts in the country to achieve the targets identified. In the new ratings by the Care Quality Commission. published in 2009, the Trust was classified as Excellent for quality of services and Good for use of resources in 2008/09.

The hospitals in Doncaster, Bassetlaw and Mexborough have worked in successful clinical and service partnerships since 1995; all hospitals have an impressive record of sustained performance and innovation. The Trust has become an established and credible hub for a wide range of specialist services alongside the major teaching and tertiary care centre in Sheffield. The Trust is an Associate Teaching Hospital of the University of Sheffield Medical School, and an Associate College of Sheffield Hallam University. The Trust became an NHS Foundation Trust in 2004. The Trust serves a population of approximately 400,000 and provides a full range of DGH-level services from five sites; Doncaster Royal Infirmary (DRI), Bassetlaw Hospital (BH), Montagu Hospital (in Mexborough), Tickhill Road Hospital (in Doncaster) and Retford Hospital (outpatient services only).

The Doncaster Royal Infirmary is a District General Hospital of 800 beds, provides a full range of medical and surgical specialties, including vascular surgery, as well as women’s and children’s services. The Infirmary has a campus style design with separate Women’s & Children’s Hospital, as well as the main hospital buildings, which house the Emergency Department and Unplanned Care Centre, Medical and Surgical Acute Assessment Units, inpatient wards and outpatient facilities.

There is a newly-refurbished and extended, well-equipped endoscopy unit providing bronchoscopy and the full range of diagnostic and therapeutic upper GI endoscopy, colonoscopy, ERCP and transoesophageal echocardiography. The Medical Imaging Department has recently been redeveloped and provides a consultant-led service for all general and specialist diagnostic and interventional imaging with digital angiography, real-time ultrasound, spiral CT scanning and MRI together with a fully equipped nuclear medicine department.

The Accident & Emergency Department was re-configured to incorporate an unplanned care facility. A modern laboratory complex houses Histopathology, Cytopathology, Microbiology, and Clinical Chemistry departments. A dedicated day case and outpatient Urology Suite operates within the Outpatients area and there is a bank of 8 operating suites with associated services on the lower ground floor of the main hospital block, with dedicated theatres for Urology and General Surgery. Orthopaedic theatres are located within the Orthopaedic Block and Obstetrics and Gynaecology theatres are housed separately within the Women’s & Children’s Hospital. A second outpatient suite opened in 2009.

ORAL & MAXILLOFACIAL SURGERY

The specialty (including Dental and Maxillofacial Radiology) has its main outpatient facilities at the Charles Clifford Dental Hospital. There is also a department at the Northern General Hospital where the maxillofacial laboratory (providing all dental/maxillofacial/anaplastology services including limb / nipple / facial prosthetics) is situated. 

Adult inpatient activity takes place at the Royal Hallamshire Hospital. The service forms a key part of the Regional Head & Neck Cancer Service, in which Sheffield is the principal partner with Chesterfield in support. Patients are referred from Barnsley, Rotherham, Doncaster and Bassetlaw.

Good interrelationships with ENT, Plastics and Neurosurgery are demonstrated by the multidisciplinary management of UAT, Salivary and Skull Base cancers. Sheffield Children’s Hospital houses all specialist inpatient and A & E services for children and is situated 250 metres from the Dental Hospital.

The service is managed within the Directorate of Oral and Dental Services based at the Dental Hospital and has for the last eight years included the OMFS in patient and out of hours emergency/ trauma service for Barnsley residents as part of a managed clinical network.  Barnsley patients access outpatient and day-case facilities locally at Barnsley District General Hospital, with inpatient care being provided in Sheffield. Paediatric OMFS facilities for Barnsley children are provided at Sheffield Children’s Hospital NHS Trust by departmental staff.

Resources include sophisticated operating microscopes, endoscopic equipment for minimally invasive procedures, CO2 Lasers x2, KTP laser, centrifugation for fat transfer techniques, implant hardware for placement of craniofacial and intraoral implants and a programme of modernisation of drill and saw surgical tools. A cone-beam CT scanner has recently been installed and commissioned at Charles Clifford Dental Hospital, allowing detailed and sectional views for surgical planning.

Recent developments include S.A.R.M.E. techniques, multi-segment osteotomies, distraction osteogenesis of both alveolar and major segments of the jaws, endoscopic brow lift, laser resurfacing techniques, limited paediatric cranial correction, TMJ replacement  by custom prosthesis, post-traumatic nasal correction, blepharoplasty, endoscopic midfacial lift and Coleman fat transfer for facial aesthetic improvement/reconstruction, collaboration with neurosurgeons in extracranial/intracranial bypass for giant intracranial aneurysm.

Related Facilities

Office accommodation, PC access and a secretarial service which is provided from a central area are all available for the consultant.

Excellent library facilities are available in the University Medical and Dental Library within the Royal Hallamshire Hospital. E-library access to journals is available to departmental members. Similarly the availability of Medical Illustration facilities is good with ready access to the Dept from a nearby location.

Experienced maxillofacial technical support/anaplastology production is provided by 2 technicians currently based at the Dept in Northern General Hospital. Routine Dental laboratory facilities are centralised within the building on the third floor of the Charles Clifford Dental Hospital.

DUTIES OF THE POST

The appointee will, together with consultant colleagues, be responsible for the provision of a comprehensive, effective & modern Oral and Maxillofacial Surgery Service, practising the full remit of the specialty whilst recognising special regional and national specialist arrangements, e.g. Cleft/ Craniofacial Services.

The successful candidate will work flexibly and co-operatively with other members of the team in order to provide a high quality, comprehensive service within this teaching hospital environment. He/she will be required to cover for colleagues during periods of absence and will have continuing responsibility for patients in his/her care and the proper functioning of the department.

Sheffield – duties in Sheffield will focus principally on working with existing colleagues in the provision of the head & neck cancer service. The successful candidate will join the Sheffield OMFS on-call rota, covering all aspects of the service on a 1 in 5 basis.

Doncaster – new partnership arrangements with Doncaster & Bassetlaw Hospitals NHS Foundation Trust involve each of the OMFS surgeons undertaking routine OMFS clinics and day case lists at Doncaster Royal Infirmary and Mexborough Montague Hospital.

The successful candidate is expected to participate in Clinical Effectiveness activities.

The appointee will be expected to take an active part in undergraduate and postgraduate teaching/training and in this respect, as long as he/she remains a consultant within the Trust, he/she will be regarded as an Honorary Senior Clinical Lecturer in the University of Sheffield, subject to the regulations of the University.

Excellent facilities and support are available to facilitate high quality research.

Regular (annual) appraisal will be part of the process, and an annual review of duties/job plan will take place with the Clinical Director as part of the job planning process.

Whilst in post the consultant will fulfil such requirements in case-mix/caseload recording and CPD/ CME to satisfy the proposed revalidation processes of the General Medical Council.

MANAGEMENT ARRANGEMENTS

The Trust’s management arrangements are centred on clinical directorates, which have been operating for many years and are well established. The Oral & Dental Services Directorate is part of the Head & Neck Clinical Care Group, which also includes Neurosciences, Ophthalmology and ENT. Clinical Directors are appointed by the Chief Executive for a three year period. Professor Ian Brook is the Clinical Director for Oral & Dental, Mr Afshin Yousefpour is the OMFS Clinical Lead.

THE MAIN CONDITIONS OF SERVICE

The appointment will be according to the Terms and Conditions of Service of Hospital Medical and Dental Staff (England) as amended from time to time. The arrangement of duties will be such as may be agreed from time to time between the employing Trust and the person appointed. 

 Whether the successful candidate chooses to accept a whole-time or part-time contract, it is agreed that any private practice undertaken, whether limited or not by the Terms and Conditions of Service, will in no way diminish the level of service that may be expected from him/her in carrying out the duties specified above (Schedule 9 Terms and Conditions of Service).

The Trust supports the concept of Continuing Medical Education. In line with Clinical Governance senior clinicians are expected to participate in continuing professional development (CPD).

HEALTH CLEARANCE & MEDICAL EXAMINATION

This appointment is subject to medical fitness and the appointee may be required to undergo a medical examination and chest X-ray.

Potential applicants should be aware of the Department of Health and GMC/GDC requirements with regard to HIV/AIDS, Tuberculosis, Hepatitis B and Hepatitis C viruses.

Candidates are advised that they will be asked at interview whether they are aware of anything preventing them from fulfilling their professional commitments and this includes health.

The successful candidate must be immune to Hepatitis ‘B’ and Tuberculosis. They will be required to provide, in advance of appointment, evidence of immunity or have a local blood test (as deemed appropriate by the Occupational Health Department).

RESIDENCES AND REMOVAL EXPENSES

The successful candidate will be required to maintain ready telecommunications contact with the main sites of clinical activity and reside not more than 10 miles by road from the Sheffield Hospitals, unless prior specific approval to a greater distance is given by the Trust.

Removal expenses on appointment will be paid in accordance with the Sheffield Teaching Hospitals NHS Trust Terms and Conditions of Service. These documents can be seen at the offices of the Medical Personnel Department. It should be noted that it is not Trust policy to reimburse interest payment on bridging loans.
