2008 Call for Submission

Applications must be received by December 21, 2008

The Catalyst Academy of Best Demonstrated Practice is comprised of professionals in the field of aging adult membership (affinity) programs. Members of the Academy select the individual(s) who will be honored by their colleagues as the current year award recipient(s). The award is presented at the Catalyst Annual Conference to one or more individuals whose practice would be characterized as exemplary and worthy of being modeled by other professional staff.

NOTE: THIS IS AN AWARD FOR INDIVIDUALS/PROFESSIONALS, NOT PROGRAMS.

The Selection Process:

1. Potential awardees must currently be involved with aging adult affinity programs or gerontology service lines or have responsibility for their oversight.

2. Potential awardees must submit an application, which addresses the series of Mandatory Considerations listed below. They may also elect to address the Optional Considerations. As long as all Mandatory Considerations are covered, the format for the application submission is up to the applicant’s discretion.

3. All submissions become the property of Catalyst Benefits, Inc and will be reviewed and evaluated by members of the Academy.

4. All Submissions must include the signature of the applicant’s supervisor or an administrative staff person higher than the supervisor.

5. The award recipient will be notified by February 1 , 2006.

6. The award will be presented at the Catalyst Benefits’ conference in April, 2008

7. The award winner will be recognized for their achievement at a staff meeting at their organization at which a representative of Academy will make a presentation to the organization’s CEO.

Selection Criteria: Award applicants must address the following Mandatory Considerations and may elect to address the Optional Considerations:

Mandatory Considerations:

• Clarity of vision, scope and depth of affinity program or aging service line.

• Strength of tie between affinity program elements and strategic initiatives of the host organization.

• Commitment to measure goal achievement and consumer satisfaction.

• Involvement with issues of aging in the greater community.

• Commitment to teaching issues and considerations of aging to other professionals in the greater community

Optional Considerations:

• Commitment to aging research.

• Elements in program that would be defined as “leading edge, beta, unique and/or “first- of -its -kind.”

• Elements in program that would be considered as setting the industry standard.

Past member, Orange County Commission on Aging and Mayor’s Senior Citizen Advisory Committee, City of Orlando; past President, International Association of Business Communicators, Orland Area Chapter.

Peter Laverty, University Medical Center, Lubbock, TX. Peter is director of UMC Health System’s “Seniors Are Special” (SAS) affinity program. SAS is located in the West Texas City of Lubbock, population 201,000. As such he is responsible for addressing the medical needs and marketing to those aged 55 and older living within a two hundred mile radius of Lubbock. SAS is a true collaborative effort between University Medical Center and Texas Tech University Health Science Center’s School of Medicine. UMC’s mission is to become the best teaching hospital in the United States. SAS was the first hospital-based senior program in Lubbock, introduced in April 1987, and currently has 15,000 members. SAS is used as a window into the hospital network, introducing members to the offices, departments, clinics and UMC Health System policies.

Susan Long, Director, Director Emeritus of Senior Services, Baptist Health System, Knoxville, TN. Industry leader in senior housing as well as the marketing of freestanding senior health centers (i.e. primary care physicians’ offices for adults over the age of fifty. Member, Board of Directors, Senior Home Companions, Knox County Senior Centers and Second Harvest Food Bank. Recipient, Catalyst Lifetime Achievement Award.

*Sue Maxwell, MSW, Gerontology Business Leader, Lee Memorial Health System, Ft. Myers, FL. Immediate past president, Southern Gerontological Society; past president, Florida Council on Aging. Faculty member, Florida Gulf Coast University School of Social Work; member of the Advisory Council, College of Health Professionals for Interdisciplinary Studies. Winner 2001 Applied Gerontologist of the Year award.
