BACKGROUND INFORMATION/GENERAL:

The purpose of this project is to provide Janitorial and Grounds Maintenance Services at the Department of Transportation’s (MDOT) Rest Areas and Roadside Parks in the University and Metro Regions.

The locations are: University Region: Carelton Rest Area, Dewitt Rest Area, Woodbury Rest Area, Reed Road Road Side Park, Frontier Road Side Park, Red Cedar Road Side Park, Berry Road Rest Area, Holt Road Rest Area, Howell Rest Area, Grand Ledge Rest Area, Chelsea Rest Area, Northfield Church, East Bound Howell Rest Area, West Bound Howell Rest Area, Grass Lake Rest Area, Okemos Rest Area, Potterville Rest Area, and Sandstone Rest Area. Port Huron TSC: (St. Clare County) - Adair Rest Area, Capac Rest Area and Richmond Rest Area.

The RFP package was placed on the Bid4Michigan website on March 10, 2011. Questions were due on March 31, 2011. The deadline for responses was April 12, 2011 @ 3:00 p.m.

If awarded the contract AMERICAN CLEANING COMPANY LLC will provide all personnel, equipment, tools, materials, supervision and other items and services necessary to perform the ground maintenance and janitorial services as described in the Specification Sheets of work for janitorial and landscaping services. Our major objective will be to maintain rest area facilities in such a manner that all those locations we are bidding in provide a clean, healthy and safe break environment for their visitors. Regarding the janitorial services and if awarded the contract AMERICAN CLEANING LLC is proud to offer its best services to STATE OF MICHIGAN on daily, weekly, monthly, quarterly, semi-annually and annually basis. AMERICAN CLEANING LLC will guarantee quality cleanliness throughout the duration of the contract. Our company will provide Deliverables/Services and otherwise do all things necessary for or incidental to the performance of work, as set forth in the Bid Janitorial Details and Tasks and Cleaning Frequency specifications (LSS). If awarded the contract AMERICAN CLEANING LLC., hereby this statement agrees to provide adequate trained personnel and a full time supervision to ensure quality services at your facilities. Janitorial personnel supplied by AMERICAN CLEANING LLC., are deemed employees of our company. While all ground maintenance work will be rendered by our subcontractor’s employees. Our site supervisor and PAM (Project Area Manager) will exercise all supervisory control and general control over all day-to-day rest area operations of our employees including control over all workers duties (subcontractor’s employees included The project management plans will identify methods, tools and processes we propose to oversee the project.

If awarded the bid and before the commencement of the contract AMERICAN CLEANING LLC will prepare for each awarded location a project work plan for implementation of both janitorial and ground maintenance services. The project plan was omitted from the proposal.

Working Supervisor will be a well trained individual who is equipped with sufficient knowledge and expertise to render a variety of janitorial assignments within the rest area facility. They report directly to PAM. The working supervisor will be based in the building throughout the State of Michigan Contract.

CLEANING TECHNICIANS (T.B.A. –Two per each location)

Their Duties include but are not limited to:

Maintain cleanliness of building including vending machine areas, main entrance ways, doors, main lobby area, outside grounds pick up, empty garbage bins, clean and check conditions of supplies throughout the day, clean sidewalks and picnic tables, storage areas, sweep mop floor, glass and partition cleaning (smudge removal), dusting, clean restrooms (partitions included), refill soap dispensers, toilet papers dispenser, sweep, mop floor and wipe equipment, door washing, graphite removal, empty trash cans, sink, urinals and toilet bowl sanitation, floor sweeping, floor mopping, clean ashtrays, dust, pick up larger objects, vacuuming all carpet areas (if applicable) and clean and shine stainless steel fixtures. If awarded the contract, AMERICAN CLEANING LLC will assign Mr. Insana as a Grounds Maintenance Manager (GMM) to oversee all ground maintenance work/schedules for all rest areas we will be awarded. Mr. Insana has a long experience in landscaping industry. His company has been involved in other joint projects with AMERICAN CLEANING LLC. Mr. Insana has been in charge of managing and implementing big and small grounds maintenance projects for our clients on behalf of AMERICAN CLEANING. Working as a subcontractor for our company, Mr. Insana has proven to be a great value and an outstanding business partner and a great team player. If awarded the contract for the locations we have bided in, American Cleaning Company LLC will subcontract all grounds maintenance services (as per location specification sheets) to GSP Properties, a specialized ground maintenance company which will perform all ground maintenance services on behalf of American Cleaning LLC.

MJV Building Services (Houseworks) will provide all personnel, equipment, tools, materials, supervision and other items and services necessary to perform the services as described in the attached Location Specification Sheet(s) (LSS) for Locations 2, 3, 4, 5, 6, 7, 9, 10, 12, 13, 14 and 15 providing service for Clinton County, Eaton County, Ingham County, Jackson County, Lenawee County and Shiawassee County. MJV Building Services (Houseworks) will also provide a telephone time keeping system at each location to alert MJV Building Services (Houseworks) if an employee is late or does not show. This system will allow MJV Building Services (Houseworks) to respond quickly to dispatch a floater person to the site and ensure each site is always manned. MJV Building Services (Houseworks) will supply a floater for each location that will be hired upon receiving award of contract. Contractor Roles and Responsibilities and Project Plan sections were thoroughly addressed. In addition, bidder provided a transition, implementation and contingency plan with their proposal.

MJV Buliding Services (Houseworks) currently has 150 Employees located throughout the state of Michigan including Berrien County, Wayne County, Kent County and several more. Michael Vollmer will be assigned as Project Manager for the overall compliance with the contract specifications and has 7+ years of experience in working with SOM on various contracts for janitorial services. Six resumes of key personnel submitted. Michael will visit each site a minimum of once a month to follow up with Site Supervisors, Site Leads and personnel on any issues, updated services and quality control. The Site Supervisors (Ann Venthouse, Tammy Garber, Phil Smith, Conny Berlanga, and Angela Johnson) will be assigned to manage the day-to-day activities of their respective locations. All have multiple years of experience managing facilities across the state of Michigan of similar size and scope of work.

On Site Leads will be will be assigned to each facility and all have multiple years of experience in the cleaning industry including facilities of similar size and scope of work.

Angela Johnson oversees our Human Resource area, and has the experience needed to ensure employees are up to date on training, new policies and procedures. Angela will be assisted by other office personnel as needed to provide updated training, follow-up with employees as needed for guidance and scheduling.

Mint Condition Cleaning

Statement of Work (Article 1) Score 30/30

Mint Condition Cleaning Service will provide all the requested services as specified in the RFP Location Specification Sheets. Mint Condition Cleaning Service will provide the personnel, equipment, materials and all other items necessary to perform the janitorial, grounds maintenance and snow removal in such a manner as to provide a clean, healthy and safe environment. Mint Condition Cleaning Service owns and maintains all equipment which

will be used for grounds maintenance, lawn care and snow removal. Mint Condition Cleaning Service will provide only competent well trained employees to perform all services in this contract. The site supervisor James Faber who is qualified and trained will be present on site as needed and required to oversee personnel and coordinate all work assignments in this contract. The supervisor will maintain the appearance, conduct and discipline of all employes which will be working at this facility. Our project work plan consists of providing janitorial cleaning and grounds maintenance according to three Rest Areas firm proposes on the LSS sheets. The supervisor will be responsible for inspecting and monitoring the quality and standards of workmanship of all work to be performed at three Rest Areas firm proposes. The supervisor will coordinate the work schedule and he will maintain the implement the employee staffing back up contingency plan as needed. The supervisor for the three Rest Areas firm proposes has the full authority to act on behalf of the company.

We will have and maintain a consistent work force of trained and knowledgeable employees. Mint Condition Cleaning Service currently maintains a staff of ten(10) employees with a 87% retention rate over the past years. For each of the three Rest Areas firm proposes four(4) employees to fulfill the regular and holiday man hours of coverage as described in the location specification sheet. We will have four(4) full time employees to fulfill the 90 hours per week during summer hours of coverage and 77 hours per week during winter hours of coverage as described in the specification sheet. We will provide separate crews for mowing and for performing the janitorial services. All personnel that are assigned to perform the janitorial duties will not perform the mowing and snow removable from parking lots.

At Your Service Cleaning

Statement of Work (Article 1) Score 30/30

AYS Facilities Maintenance will supply everything necessary to provide the services described in the LSS. This will include equipment, supplies, tools, all personnel, supervision, main office support, quality control inspections, etc. AYS Facilities Maintenance will provide all items necessary to perform the required work. It will be our responsibility to manage the ordering and delivery acceptance of all supplies and materials. Ordering will be done by the Site Supervisor and supplies will be delivered during normal work hours when an attendant is on site to accept and put away deliveries. AYS Facilities Maintenance will provide and be responsible for all contractor staff. Personnel as follows:

Project Manager – At contract start up the project manager would be David Mroz. David is President of AYS Facilities Maintenance. He will oversee the implementation of all contracts awarded to the company. He has been with the company for 5 years and has several additional years of executive and project management experience.

Site Supervisor – At contract start up the site supervisor will be Pamela Root. Pamela is General Manager of AYS Facilities Maintenance. She will provide a supervisory role, but will not be a member of the janitorial staff at the locations. She will be the company representative at the site and holds a decision making position. She will be responsible for day-to-day operations, ordering supplies, maintaining adequate staffing, training staff, quality control, etc. She will also be responsible for the hiring, firing, and discipline of all site attendants. The site supervisor will spend time each week at each location.

Staff Support – Staff support will come from the Site Supervisor and the company’s home office located in Flint, MI. This will include Human Resources, Timekeeping and Payroll, Warehousing and Supplies, MSDS and OSHA compliance and training, workers compensation and insurance issues, unemployment issues, and general office staff.

Site Staff Attendants – Site staff will be hired from the general vicinity of the area to be serviced. Hiring will be done immediately after award of contract. Site Staff will report directly to the Site Supervisor and will have unlimited contact with the AYS office. Their function will be to provide service as indicated per the site specifications. Site staff attendants starting wage will range from $7.50 per hour to $8.00 per hour. Number of hours to be worked at each location will coincide with the number of hours required to be worked, which is indicated on each individual LSS. Additional hours will be worked on holidays as specified.

Subcontractors – If subcontractors are used at any location, they will be hired from the same general vicinity of the area to be serviced. They will report directly to the Site Supervisor.

To implement this project, AYS will first meet with the CCI or specified representative to once again survey the site and review specifications. It is important at this point to recognize particular facts about the site, and environment. We would schedule a pre-start meeting with the CCI to work out all details and survey the area available to us for material and equipment storage, if applicable.

We would like to continue to use Out of Sight for lawn maintenance if awarded the following locations he has serviced in the past: Northfield Church, Chelsea, EB Howell and WB Howell. We would also like to be able to have Out of Sight service any additional locations we may be awarded.

We have all necessary equipment to do the janitorial work as well as the equipment to do the grounds maintenance. We will provided the necessary personal to perform the work set forth in the scope of work to meet or exceeded the satisfaction of the state. We understand that each rest area may have different hours of coverage. We will have extra personnel on call in case of emergencies and have the extra personal to cover the holidays. We will adhere to the cleaning duties for daily as follows for the restrooms daily between 7am and 8am

If awarded the contract we will supply the state with the names and phone numbers of specific employees and the project manager, site supervisor. The support staff will depend on the location of work. The project manager will oversee the entire contract, the site supervisor shall be a qualified and trained on a full time basis and will be designated I writing as the contractors representative at the rest area site any person as an attendant is not considered a supervisor we will exercise all supervisory control over all workers. At the conclusion of each service we will inspect the facility for completion and quality of work and correct anything that was not done right or missed. For each location we will supply a work plan to cover the schedule of operation, man hours, the daily work schedule of the tasks that need to be performed on a daily basis and a project management plan to identify the methods, tools to oversee the project and address any concerns or changes that might apply. To be in constant communication with the agency or department project lead. See attached list for equipment for snow plowing and lawn maintenance.

It is the intention that bidder provides the state the highest quality of trained personnel. All personnel will be trained before they are to work on their own. Bidder has a staff of three or four people to cover the hours. There will also be a person on call in case of emergencies and will have at least four extra people for the holidays. The supervisor that will be in charge of this project will have over eight years of experiences in the landscaping and property management and janitorial business. Bidder intends to staff the rest area with at least 4 attendances not including the project manager and supervisor and has extra personnel on staff for holidays. Customized Cleaning Services

Statement of Work (Article 1) Score 20/30

The cleaning staff firm intends to deploy will include only those with prior experience in the janitorial field. The employees for this site will have experience in cleaning restrooms and public areas and maintaining them as presentable at all times. They must and will meet Customized Cleaning Services employment standards and will agree to our no use drug policy. They are required to have or will be furnished a cell phone in case of emergencies and in order to communicate consistently and on a timely basis to our office staff. Customized Cleaning Services does not currently have a disabled person on our staff. In the event that this takes place we will comply by all state and federal laws as well as make sure the employee is able to properly perform the duties necessary for the performance of work, as set forth in Attachment A and Location Specification Sheets (LSS)? Over past 20 years bidder has been able to maintain a higher standard in the industry as performed by staff? Bidder is a GSA contractor and performs various floor and floor maintenance processes all over the state in various private, county, state and federal facilities. In the state of Michigan we are at this time a single source provider for SaniGLAZE and One Step epoxy floor system. We acknowledge and agree to an audit that may be required annually and understand that it may be necessary for the performance of this contract?

We expect there will be a need for 2 staff members per day during the long holiday weekends. The standard shifts will most likely be 12 hour days or the hours as per the schedule requirement. For back-up hiring we will interview candidates that are listed with the Michigan Works program if they can show prior janitorial experience or can provide ample signs of desire to work and maintain employment. Customized Cleaning Services, Inc will use our existing supervisory staff that has been employed by us for more than 2 years to handle all oversight and supervision. We will be happy to provide a full staffing plan with all request upon receipt of contract?

Operations Manager, King Anderson has a degree in Psychology and Mental Health. He was formally a sale manager at an office for Oak Street Mortgage. Prior history also includes working as a sub- contractor in construction for 5 years and operating a farm for 10 years.

Owner Operator, Daniel Waswick started Customized Cleaning Services in 1991. He graduated from MSU in 1979 with a degree in mechanical engineering. Prior to starting CCS he was general manager of General Tool and Die located in Grand Rapids, MI.

Statement of Work (Article 1) Score 30/30

Hi-Tec Building Services, Inc. agrees to provide all personnel, equipment, tools, materials, supervision and other items and services necessary to perform the services as described in the Bid Specifications and attached Location Specification Sheet(s) (LSS). For each location, the assigned Operations Managers specified in 1.030 Roles and Responsibilities, A. Personnel will oversee all operations of the services to be implemented at the Rest Areas and/or Roadside Parks including training and OSHA/MIOSHA. The Operations Manager will meet with the CCI and other agency or departmental project leads, on a basis to be established by CCI and Hi-Tec, but shall meet quarterly, at a minimum, for the purpose of reviewing progress and providing necessary guidance in solving problems that arise, and to continuously communicate. Operations Managers will complete quality assurance checks to ensure performance and quality standards are being met. If performance and/or quality issues exist, the Operations Manager will evaluate the situation and may require additional cleaning time built into the schedule and/or the re-training of the cleaning team member(s). The Operations Managers will be involved in inception meetings covering new hires, training, inventory, MSDS sheets, etc. with Hi-Tec’s support staff.

HTA will provide staff and equipment needed to perform work at each site according to contract specifications. HTA Companies, Inc. agrees to comply with all sections of the RFP and the agency and CCI as stated above.
