For your convenience, Western Connecticut Health Network has multiple Patient Service Centers for specimen collection (listed below) to help meet the needs of our patients.

We provide appointments as well as walk-in phlebotomy and specimen collection services.

Important Information for Patients

In order to expedite your service, please be sure to have a requisition that has been completely filled in with the following information. Please keep your requisition and paperwork legible. Paper requisitions must be signed by the ordering physician.

Patient identification required, photo ID is preferable Insurance information: please note, we may copy your insurance card with each visit Please arrive at the specimen collection facility at least 15 minutes prior to closing time listed above. For collection of specimens where fasting is required, please do not eat anything after midnight. Testing for lipids profile (cholesterol, HDL, triglycerides) and fasting glucose requires a 12-hour fast. If you have a history of fainting, please bring a family member or friend and be sure to notify your care giver prior to procedure.

In-Home Phlebotomy Services

The patient who qualifies for phlebotomy in the home is the patient who is frail of health and is unable to drive or be brought to a blood collection center where phlebotomy can be performed. Patients able to meet medical appointments or social engagements i.e. hair appointments, shopping, etc., are NOT eligible for home care services.

Home Care services are not available on a STAT basis and are scheduled as needed the "week of". Phlebotomy services are available for homebound patients Monday through Thursday. Home Care visits must be scheduled in advance by calling 203-739-7643 and by faxing a completed Home Care Booking Form to 203-739-8835. Questions regarding this service may be directed to the Operations Specialist, Specimen Acquisition and Processing at 203-739-7538.

Report Delivery

Electronic Reports Electronic transfer of test results is provided via interface into a number of on-line systems, i.e. LifePoint, HealthLink, Allscripts, etc. These electronic systems allow for immediate availability of test results as soon as results are verified by the Main Laboratory.

Faxed Reports Electronic transfer of test results is provided via the Laboratory Information Systems to a fax machine. Faxed reports are sent in batch mode at specific time intervals throughout the day. This system is set up for timely delivery of test results when orders are non-electronic.

Mailed reports Mailed reports are provided when electronic report delivery is not available or on request.

Specimen Pick Up and Delivery Service for Physician Offices

Courier services are available for transporting specimens to the Danbury Hospital Laboratory located at the Network's main campus. Regularly scheduled courier service is available throughout the greater Danbury area Monday through Friday 9:00 a.m. to 9:00 p.m. and Saturday 9:00 a.m.. to 5:00 p.m. Routine pick-up frequency is determined by volume. STAT requests are available with an expected 2-hour turnaround time from the time the call is received to the time of specimen pick-up. requests must be called to the Laboratory's main number 203-739-7306. There are no Courier services available on Sundays or Holidays. Couriers are trained in the proper transportation of specimens to maintain their integrity under controlled conditions. Supplies and other necessary materials are also distributed by our Courier.

Supply Request Forms

CMS regulations require that we provide supplies to only those offices that send specimens to Danbury Hospital Laboratory. The Laboratory may supply only regulated supply equipment. To request supplies, please use our Laboratory Supply Requisition Form. For those offices using LifePoint, please go on line to Laboratory Orders and click on Order Supplies.
