MAILING INSTRUCTIONS FOR NEW MEMBER/RENEWAL FORMS
(Do not bring form to Human Resources or to any BJ’s Club locations.)
Open a new Inner Circle Membership for $25/12 months or a New Rewards membership for $50/12 months. (New members only…previous or existing members not eligible.)
OR
EXTEND your existing Inner Circle membership for $40/13 months ($15 savings/1 free month) or your existing Rewards membership for $80/13 months ($30 savings/1 free month).
With payment, 13 months will be ADDED ONTO expiration date of existing members.
*Form may also be scanned as a pdf to Dlisonbee@ if paying by debit or credit card.*

at top of form. New members leave this field blank.
2. Please fill out primary name, address, phone, email address** and information
for free second household card. (Email required on NEW member forms.)
3. Sign in the Primary Signature field.
4. Include payment of $25 or $50 if opening a NEW membership.
Include Payment of $40.00 or $80.00 if extending your membership.

5. Place application and payment in envelope and send to the address above.
Note: Membership forms will be processed within two days of arrival. Please contact Debbie Lisonbee (Dlisonbee@) with any questions regarding this program. Please expect less than one week for processing if email address is provided. New members will receive their activation form via email (check spam file too) which they will present to the membership desk at any BJ’s Club in order to receive their permanent cards. New members who do not provide their email address will receive their activation form in the mail which they will present to the membership desk at any BJ’s Club in order to be issued their permanent cards. Please allow an additional week if email address is not provided. Renewing members will receive renewal confirmation via email only and will notice extended membership date reflected in the expiration date field on BJ’s store receipt.
