Thank you for submitting your transcript to Evangel University. If you are/were home schooled, you will need to submit a high school transcript as part of your Evangel application. All transcripts must be typed (we cannot accept hand-written transcripts). This template is provided for your convenience and is acceptable as an official transcript. For transcripts submitted prior to graduating from high school, you may leave the graduation date field blank. In order to be a "final" transcript this form must include the exact date of graduation as well as a final cumulative GPA. You may use this template as the initial transcript, then update and submit it again after you have officially graduated. This form is fillable on your computer. In order to utilize the fillable functionality you will need to save it to your computer after completing the form. Please submit the completed file via email to Admissions@evangel.edu.
