VITEM V – RN 100/2013 - Requirements check-list:

(work visa for applicants assigned to go to Brazil to provide short term technical assistance or technical training due to a technology transference or technical maintenance contract between a Brazilian company and a non-Brazilian company)

STANDARD REQUIREMENTS FOR ALL VITEM V VISAS BASED ON RN N.º 100/2013

□ A) Applicant's Passport: original, undamaged; valid for at least another 6 months, with at least two blank visa pages.

□ B) Visa Application Form: printed receipt of the visa application form filled out and successfully submitted online duly signed by the passport holder (or parents/legal guardian of a minor) and with the photo glued on the appropriate boxes.

□ C) Photograph: one recent, clear, front view, in color, professional 2"x2" passport photo of the applicant with a plain white or off-white background.

□ FEES: Individual money order from the United States Postal Service in the exact amount for each visa requested listing applicant’s name on the section “From”.

Example: John will come in person to submit 2 visa applications: 1 for himself (American) and 1 for his Norwegian coworker. During his appointment he must have 2 USPS money orders: one from him in the amount of US$ 290.00 (US$ 100.00 basic visa fee and US$ 190.00 reciprocity processing fee); one from his coworker in the amount of US$ 100.00 (US$ 100.00 basic visa).

□ G) Copy of Brazilian company's registration with the “Cadastro Nacional de Pessoas Jurídicas” (CNPJ).

□ M) Original Notarized Letter from the sponsoring company in Brazil, on their letterhead stationery and signed by a senior manager (original signature acknowledged by a “Cartório”) stating the following information:

□ Name, CNPJ, address and nature of business of the Brazilian company;

□ Senior manager of Brazilian company’s name, title and capacity to act on behalf of the Brazilian sponsoring company;

□ Name(s) of person(s) to be contacted in Brazil in case the Consulate needs additional information, their phone number(s) and function at the Brazilian company;

□ Detailed information concerning services to be rendered to the Brazilian company;

□ Name and address of the company in the US contracted by the Brazilian company to provide technical services.

□ Name, phone number and email of manager of the company in the US in case additional information is deemed necessary;

□ Full name, nationality, passport number, profession and brief description of the qualification (education and experience) of the applicant;

□ Expected travel dates of trip to Brazil;

□ Responsibility of the Brazilian company for applicant's stay in Brazil and his/her return to the country of origin;

NOTES:

The letter from the Brazilian sponsor MUST HAVE ALL of the above mentioned information. Letters missing any of the required information and/or without original signature WILL NOT be accepted and therefore visa application WILL NOT be accepted.

The Consulate may request documented proof that the company in the US is formally registered with local authorities;

OTHER REQUIREMENTS FOR A VITEM V BASED ON RN 100/13

If the visa application will not be submitted in person:

□ H) Proof of jurisdiction: notarized copy of driver’s license, utility bill, lease or bank statements.

If the not applying with a US passport:

□ K) Proof of Legal Residency: notarized copy of visa, permanent resident card or US passport or US birth certificate.

If you will be applying for a visa on a passport issued by Afghanistan, Central African Republic, Iran, Iraq, Jordan, Kosovo, Lebanon, Libya, Nepal, Nigeria, North Korea, Pakistan, Palestine, Senegal, South Sudan, Sudan, Syria or Taiwan:

□ I) Detailed information on applicant's place of lodging in Brazil (hotel reservation or letter from Brazilian host), flight and notarized bank statements reflecting the past 3 months are required.
