Abstract:

The Health and Human Services Delivery Center (HHS DC) supports the Departments of Health (DOH), Human Services (DHS), Aging (PDA), Drug and Alcohol Programs (DDAP) and Military and Veterans Affairs (DMVA).

General:

Each site at the Health & Human Services Delivery Center (HHS DC) connected to the HHS DC network may need upgrades and performance improvements to its site network. The HHS DC, Health & Human Services Delivery Center contracts vendors for site-specific network surveys and upgrades. It creates and submits a statement of work (SOW) to vendors to do the survey(s). A chosen vendor that agrees to the SOW performs the network survey(s).

The deliverable expected of the vendor doing the network survey(s) is a Network Design Report (NDR) for each site surveyed. The vendor uses the network assessment information in this document to create one NDR per site-specific, network survey.

See Example of a Statement of Work to a Vendor for a Network Survey and Analysis for an example of an SOW for a network survey.

See “Appendix I: ” for assistance completing this template.

The Network Assessment starts at the “Network Design Report” section and runs to the “Appendix I: Completing the Network Design Report” section of this document.

The purpose of this document is to provide instructions for creating an NDR.

Procedure:
