|The Site-based learning programme will focus mainly on people-centred primary care. The current primary health care(PHC) movement includes four sets |
|of PHC reforms for reducing inequalities and improving health for all, as documented in the World Health Report 2008: Primary Health Care: Now More |
|Than Ever. Service delivery reforms are about re-organizing health services around people’s needs and expectations, so as to make the more socially |
|relevant and more responsive to the changing world, while producing better outcomes. |

|The following questions are central to the activities within the site-based learning programme and answers on these questions will add to the existing|
|knowledge on the issue of how to tackle health disparities in a population by appropriate Health Care. |
|What are the basic skills and competences of a Primary Care health professional to function in a Community Oriented and People Centred Primary Health |
|Care system |
|How can health centres be community oriented and people centred and contribute to social inclusion? For example, how do they ensure that access to |
|services is equal for both genders and for minority groups? |
|Primary Care teams: how far have they been developed and how do they function in practice; how can benefits be described and assessed in particularly |
|regarding social inclusion? |
|What have been the developments in terms of new professions and distribution or delegation of responsibilities within Primary Care settings? What are |
|minimum levels of collaboration to ensure involvement in tackling social exclusion problems at community level? |
|What obstacles in terms of legislation and regulations for Health Centres to act on community level had to be overcome? |
|How can national health systems contribute to PHC-development? |
|Answers on these questions will also help to shift the research from describing patterns of health disparities to evaluating the effectiveness of |
|policies and interventions at: |

|• Policy level |
|in order to address disparities. |
