Human ResourcesHR-XX-XX-352425-80454500XXJob titleDementia Friendly Communities CoordinatorReports to(Insert Title) Job purposeThe objective of the Dementia Friendly Communities Coordinator is to coordinate the development of the Dementia Friendly Community Proof of Concept Project in the designated community. The Dementia Friendly Communities Coordinator will have overall goals and key deliverables outlined respectively below:To build community capacity to support individuals to age-in-place and remain the community for as long as possible.To raise awareness of dementia within the wider community with the aim to reduce stigma associated with dementia. Duties and responsibilitiesPartnerships and CollaborationsTo facilitate the creation of dementia friendly communitiesBuilds credibility, develops positive relationships and collaborates with key internal and external stakeholders and multi-agencies to take the Dementia Friendly Communities Proof of Concept project forward in the designated communityWork in consultation with people living with dementia and their caregivers to create a dementia friendly community to identify priorities in creating dementia friendly environments in the local communityRegularly liaises with the Dementia Friendly Communities Coordinator Regularly liaises with the Manager (who has oversight/project management to the Dementia Friendly Communities initiative)To coordinate and manage the local community meetings, including setting agendas, creating an action plan, booking a venue, distribution of meeting notes etc. Attends monthly meetingsCreates a draft work plan Identifies initial community supports and resources available in the communityLiaises with key stakeholders including library, community association centres and other locations to identify potential spaces for education sessions in the communityTo create a framework in which local community activities, meetings and events are structured so that they can be self-sustaining once the support of the post is withdrawnProject ManagementTo promote the work both locally and nationally and support the development of dementia friendly communitiesTo undertake any other duties or projects with the nature and grade of this post as requiredEducation and TrainingReviews toolkits from other areas to support development of training materialsCollaborates with key stakeholders to develop training materialsCarries out surveys of businesses and other local community organizations to gauge interest in trainingFollows up with interested business and other local community organizations for trainingCreates training calendarsTracks local businesses and other organizations who would like training and have received trainingOrganizes and executes relevant training sessions in the local communityPromotes better understanding of dementia across communities, in order to reduce stigma and discrimination and promote independence for people living with dementiaShare best practice as well as support dementia awareness raising activity and/or trainingResource DevelopmentIdentifies champions for carrying out further training sessions to support sustainability of the Dementia Friendly CommunityEngages and develops a network and database of volunteersCommunication/Knowledge TranslationUpdates Communications plan Facilitates knowledge dissemination to highlight progress made on the Dementia Friendly Communities initiative through writing items for newsletter or other communication mechanisms; this could include a monthly Dementia Friendly Communities bulletin for website and other websites deemed appropriateWorks in collaboration with multi agencies to organize events which will raise the profile of the Dementia Friendly Community Proof of Concept project.Occasional evening work giving presentations to interested parties eg: Community AssociationsCollaborates with the Communications department (if applicable) to develop materials for distribution to the media and the Okotoks community Develops resource library in collaboration with members of coordinating committeeGovernanceReports project progress to the Advising committee (if applicable)Evaluation and SustainabilityDevelops evaluation plan Establishes systems for monitoring and evaluating the work of the communityTo report evidence of good practice, success stories and areas of risk Develops survey questions Writes final report To ensure communities are set up and sustainable once the support of the Coordinator post is withdrawnAccountable for monitoring compliance of communities against locally set objectivesAdditional ResponsibilitiesTo comply with data protection regulations, ensuring that information on specific individuals remains confidentialQualificationsEducation: Bachelor’s degree requiredKnowledge of dementia and dementia friendly communitiesExperience and Attributes:Strong relationship and partnership building skills with internal and external stakeholders that foster collaboration and value added networksExperience collaborating with multi-agenciesStrong planning and problem solving skillsExceptional communication and presentation skills (confidence in public speaking)Proven skills in developing and implementing successful new servicesKeen interest in Research and Innovation, Senior’s Health and dementiaProven ability to perform at a high level strategically and operationallyManage competing deadlinesCommitted Excellent teamwork skillsAn understanding of the needs of people living with dementia and their caregiversMarketing and negotiating skillsExcellent report writing skillsGood time management skills, ability to plan, prioritize and deliver in tight timescalesCompetencies:Relationship buildingFacilitationProblem solvingCommunicationOrganizedProactiveFlexibilitySelf-starterInnovationEmpatheticLeadershipPlanning Strategic thinkingAllocation of Time70% Community Related Responsibilities30% Administrative Related ResponsibilitiesWorking conditionsApproximately 20 – 30 hours per weekWorking in the evening may be requiredWillingness to travel within the defined geographical area of the rolePhysical requirementsOffice Environment: Most of the time is spent sitting or standing in same location with liberal opportunity to move about.
