Rule 3.6 – School Information Sheet & Code of Ethical Conduct Agreement

The lead teacher coach must complete and submit a School Information Sheet (Word version) with team specific information prior to the regional competition by the deadline provided by the SC Bar. Every participant must be included on this form. Any additions or deletions to the team must be resubmitted to the SC Bar prior to the competition.

Description

The School Information Sheet is combined with the Code of Ethical Conduct Agreement Form for ease of completing two forms at once. The School Information Sheet serves the purpose of providing names of every participant (teacher coaches, attorney coaches, and students). This information is merged exactly as typed and used for planning, press releases, certificates of participation, etc. The Code of Ethical Conduct Agreement requires each participant to read the agreement and sign by their name indicating he or she will abide by the Agreement. The signature also confirms the correct name (i.,e. not a nick name) and spelling of their name is correct. (See samples in the next few pages.)

Roles are not assigned on this form. The Team Roster Form (also found in the Forms Section), asks for the roles assigned to each student and the order of calling witnesses.

Directions

1) Complete the upper and lower portion by typing the information in the Word document. Teams with more than 18 students will require a second form to be completed.

2) Save the Word Document with your school’s name, i.e. Meadow Academy School Information.

3) Email the Word Document (no PDFs) as an attachment to Marian Kirk at mkirk@ by the deadline provided.

4) Print the completed form with the typed names. (First Name / Last Name – alphabetical order by last name)

Correct listing looks like Jane Doe, not Doe, Jane. Not all caps please.

5) Have all of the teacher coaches, attorney coaches, and students read the Code of Ethical Conduct Agreement and legibly sign by their typed name in pen – no pencils.

6) Submit the signed form via fax to Marian Kirk at (803) 771-0419 or scan and email the document to mkirk@ by the deadline.

Questions/Answers

What if I add someone after the form has been submitted both ways?

Email Marian Kirk (mkirk@) with the addition. Then have the person sign on the same form previously signed by the team participants with their name handwritten clearly on the sheet. (Note that changes after the deadline are not guaranteed in any publications.)

What if I lose someone after the form has been submitted?

Email Marian Kirk (mkirk@) with the deletion.
