II. Community Service (Please notice 2013 updates)

Service hours must be performed without pay, compensation or reward and with adult supervision. Mandatory or court ordered community service does not apply. Samples of acceptable volunteer service activities are listed below. Strive for activities that benefit the community at large and not only your personal, school or church family life. You may count hours that you earn through your church and school, but please make sure they align themselves with the SOM guidelines.

IT IS YOUR RESPONSIBILITY TO MAKE SURE YOUR PROJECT QUALIFIES FOR SERVICE HOURS. If you have any question about your volunteer hours qualifying for Spirit of Midway, please contact PTA chairperson, Patrice Wills, Patrice@ for verification. You are encouraged to submit additional volunteer hours so that if any or your submitted service hours are not eligible, the committee can consider additional hours in order for student qualification.

The Spirit of Midway Award requires at least 15 hours of community service per calendar year. Sharing your time and talents for the benefit of others is an important component of the award. Service activities should be performed without pay or compensation and under adult supervision. Mandatory or court ordered community service will not apply. AT LEAST TWO DIFFERENT PROJECTS BENEFITTING THE COMMUNITY AT LARGE MUST BE DONE FOR THE 15 HOUR TOTAL. Hours must be completed between January 1, 2013 and December 31, 2013. Community service for incoming freshmen and new students to MHS will begin June 6, 2013. The form below must be completed even if you attach separate documentation.

Make copies of this form as needed for community service hour verification. This page MUST be completed even if you attach a signed volunteer form from an organization. Use one form per activity. Please do not group different service activities together. Additional hours may be submitted. In the event that ineligible hours are submitted, then the student’s additional work can be considered instead.
