As you may be aware, the new South Glasgow University Hospital and new Royal Hospital for Sick Children are due to open on the current Southern site early in 2015.

With this in mind, please note that positions based within the Victoria Infirmary, Mansionhouse Unit, the Western Infirmary and the current Royal Hospital for Sick Children at Yorkhill will change location and move to the new hospitals.

Gartnavel General Hospital and Glasgow Royal Infirmary will also have some services affected by moves to the new Hospitals.

We welcome applications for a replacement post of Consultant Congenital Cardiac Surgeon based at the Royal Hospital for Sick Children, Glasgow and the Golden Jubilee Hospital, Clydebank. The successful applicant will join the existing team of cardiac surgeons to provide the national paediatric and adult congenital cardiac surgical service for Scotland.

 

The Royal Hospital for Sick Children is close to the centre of the city and the Golden Jubilee National Hospital is approximately seven miles away and both have excellent transport links and easy access to the Highlands and Islands of Scotland.

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Cardiac Surgery or eligibility for specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training.

JOB DESCRIPTION

Applications are invited for the post of Consultant Congenital cardiac surgeon at the Royal Hospital for Sick Children, Glasgow and the GJNH, Clydebank.

The successful applicant will join a team of 3 other consultant Cardiac Surgeons delivering the Scottish Paediatric and Adult congenital heart surgical service. The Senior Surgeon will retire in 2017. It is anticipated that this is a replacement post. Support and mentoring will be provided as required.

This paediatric cardiac surgical centre is not part of the current reorganisation of surgical services in England and Wales.

Section 1 Hospital Paediatric and Neonatal Services across NHS Greater Glasgow and Clyde and the Golden Jubilee National Hospital Clydebank

The majority of acute children’s services in NHS Greater Glasgow and Clyde are provided from the Royal Hospital for Sick Children (RHSC). A paediatric ward and neonatal unit in the Royal Alexandria Hospital supports acute care for children and young people and neonates in the Clyde catchment area of the NHS Board. Neonatal intensive care is provided at the Southern General Maternity Hospital, Princess Royal Maternity and Royal Alexandria Hospital. The RHSC will be re-located to a new build on the Southern General Hospital campus in 2015.

The Royal Hospital for Sick Children Yorkhill

RHSC is the largest paediatric teaching hospital in Scotland. It provides care for children resident within the Greater Glasgow area, acts as a tertiary referral centre for children from the West of Scotland and in a range of subspecialties including invasive paediatric cardiac services, from the whole of Scotland.

• All paediatric medical and surgical subspecialties are provided, including general medical paediatrics, respiratory, endocrinology, gastroenterology and nutrition, haemato-oncology (bone marrow transplant service), neurology, neurosurgery, cranio facial and maxillofacial surgery, nephrology, cardiology, immunology and infectious disease, dermatology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, rheumatology and orthopaedics, general paediatric and neonatal surgery

• There are 173 inpatient beds excluding paediatric and neonatal critical care, day care and medical assessment with cardiac patients located in ward 5a (24 bed ward)

• There is a 22 bed paediatric intensive and high dependency care unit. Co located with this is further 18 neonatal intensive care cots

• There are 7 full operating theatres, day surgery unit, pre assessment team, dental suite and endoscopy room in the main theatre complex in the hospital. Cardiac surgery has a dedicated theatre and team providing 24 / 7 cover to their patients.

• There is a full radiology suite of MRI, catheterization laboratory, CT scanner and clinical physics department. There is also an extensive cardiac physiology department

• National services provided from RHSC include:

o Cardiac Surgery and interventional cardiology

o Neonatal cardiology,

o Neonatal ENT,

o ECLS

o Congenital Diaphragmatic Hernia network,

o Vein of Galen assessment and treatment (together with Great Ormond Street as a UK service).

• Large outpatient, medical day care, medical assessment unit and emergency department

• On site paediatric and neonatal transport/retrieval services

• A pan Greater Glasgow Child Protection Unit has been developed on site.

Laboratory services to the RHSC are provided from the Southern General Hospital campus.

RHSC provides the major Undergraduate Paediatric Teaching facility for the University of Glasgow and accommodates the University Departments of Child Health, Child and Family Psychiatry, Medical Genetics, Human Nutrition, Paediatric Pathology, Paediatric Biochemistry and Paediatric Surgery.

Southern General Maternity

At the start of 2010 the new neonatal unit opened above the labour ward on the Southern General site with projected delivery numbers for the hospital of 5,900 births per year. Because the Ian Donald Fetal Medicine unit is on the same site a significant proportion of the neonatal workload involves delivery of infants with known congenital anomalies. Limited radiology support is provided by colleagues from the RHSC but infants requiring invasive procedures or complex imaging are transferred to the RHSC neonatal unit. There is physical space for 34 intensive care and high dependency cots and 26 special care cots.

When the new children’s hospital opens on the Southern general site in 2015, all neonatal services currently provided in the NICU at RHSC will move over to the Maternity unit. There will be a physical link to the intensive care floor of the new children’s hospital.

There is a dedicated fully equipped echo machine and remote telemedicine link on site together with access to the cardiac patient database kept at RHSC.

Princess Royal Maternity (PRM)

The PRM is a large maternity unit, situated about 1 mile east of the centre of Glasgow. It is on the campus of Glasgow Royal Infirmary, which provides services for the population of the East and North of Glasgow. It is a purpose built 120 bed maternity hospital, with a capacity for 6,800 deliveries per annum. There are 10 intensive care cots, 23 special care cots and a 4 bed transitional care unit. All neonatal intensive care can be provided except for those infants who require neonatal surgery and/or extra-corporeal life support services (ECMO). A telemedicine link with the Royal Hospital for Sick Children has been long established allowing live consultation and the transmission of ultrasound images. The University Chair of Obstetrics and Maternal Medicine is based on site and there is an active research philosophy in the unit.

Golden Jubilee National Hospital (GJNH)

The GJNH is located 8 miles west of the city centre along the river Clyde. It contains the West of Scotland regional and national heart and lung centre which provides cardiac surgery, thoracic surgery and interventional cardiac services to the population of the West of Scotland. It hosts designated National services: the Scottish Adult Congenital Cardiac Service, the Scottish Pulmonary Vascular Unit and Scottish National Advanced Heart Failure Service (including heart transplantation). There is a well developed ECLS programme. The adult congenital unit performs over 200 adult congenital operations and catheter interventions per year and is closely integrated with the paediatric congenital cardiac service.

Section 2 New Royal Hospital for Sick Children

The construction of a new children’s hospital (replacing the current Royal Hospital for Sick Children) is nearing completion on the Southern General Hospitals Campus alongside a new adult hospital and existing maternity unit including a new neonatal unit.

Commissioning and functioning of this new tripartite clinical arrangement for NHS Greater Glasgow covering acute care of adult, children and maternity services is expected to be completed by mid 2015.

Critical Care Services in RHSC

The Paediatric Intensive Care Unit (PICU) is one of two centres for the care of critically ill children in Scotland admitting 650 – 700 children annually including 80 transferred from other hospitals throughout Scotland. It is the only unit dealing with post cardiac surgical patients and ECMO. The National Paediatric Critical Care Transport Service is run jointly with Edinburgh’s Royal Hospital for Sick Children. The Unit is funded for 7 cardiac intensive care beds which are run on a flexible basis to accommodate the varying workload. It is one of the four extra corporeal Life Support centres for the UK. An adjacent High Dependency Unit increases the flexibility of the critical care service.

Critical Care Services in GJNH

There are 38 Critical Care beds occupying 4 co-situated units, these are managed nominally as 19 ICU beds and 16 HDU beds, but there is internal flexibility dependent on workload. Together with postoperative cardiac surgical patients the unit manages critically ill patients with heart failure undergoing heart transplants through the Scottish National Advanced Heart Failure Service. Primary Cardiology Intervention patients are supported together with Thoracic work. There is an established ECLS support programme including ECMO and VADS..

The Cardiology service is supported by 4 WTE secretaries. The Cardiac Information System is supported by 2 WTE Cardiac data coordinators, allowing the ability to produce detailed activity and audit data.

The Department enjoys the services of highly skilled nurses working across, Cardiac surgery/Cardiology ward, Intensive Care, Neonatal Intensive Care, Cardiac Theatre, Catheter lab and pre-admission service. There are both Intensive Care and Cardiac specific accredited Nursing Courses in place. A Cardiac Services Coordinator manages a lead nursing service and patient support services and quality.

The service is further supported by a team of 4.3 cardiac liaison nurses and 2 cardiac nurse practitioners.

The cardiac theatre team including anaesthesia, provide dedicated cardiac surgery cover out of hours. The catheter lab, radiography and cardiac physiology staff provide an out of hours on call service for interventional cardiology.

Cardiology

Junior staffing in the cardiology department is provided by a resident on-call 1 in 8 rota populated by a combination of research fellows, fellows attached to the advanced heart failure and adult congenital services and 3 CCU fellows In addition, cardiology trainees in the West of Scotland in year ST4 and above rotate through the CCU and provide middle grade out-of-hours cover. Finally, sub-specialty trainees undertake modular training in the GJNH during their final two years.

ix) Nursing Support (GJNH)

The Department works with a team of highly skilled nurses working across, the wards, the Critical Care areas, ac Theatre, Catheter lab and the Out Patients department.. The service is further supported by teams from Perfusion Physiotherapy and additional AHP staff. There are a number of practitioners in advanced practice roles, including:Advanced Nurse Practitioners, Specialist Nurses, Cardiac Rehab staff, Specialist Education staff and research staff. The GJNH is focused on Quality and Improvement, with all areas of care delivery focusing on the needs of the patient using a multidisciplinary approach

The cardiac theatre team, provide dedicated cardiac surgery cover out of hours. The catheter lab, radiography and cardiac physiology staff provide an out of hours on call

(c) Duties of the Post:

(i) Clinical

The successful candidate will have, or be within 6 months of obtaining, a CCT in cardiac surgery or equivalent and expected to practice as an independent paediatric cardiac surgeon. He/she will be committed to providing paediatric cardiac surgical services in Greater Glasgow and Clyde and adult congenital cardiac services in GJNH covering gaps in the service if they arise. This is a new post and it is expected the successful candidate will bring skills that will complement those already present within the unit.

Clinical duties will include ward rounds; out patient clinics; outreach clinics; liaison with cardiac surgical and critical care colleagues. The post holder will provide continuous on call cover of the ward areas as well as support to intensive care and surgical services during ‘on service’ weeks.

(ii) Teaching

The successful applicant will be expected to take a role in undergraduate and postgraduate teaching. He or she will also be expected to be responsible for the professional supervision, training and development of doctors in training grades.

(iii) Research and audit

The successful applicant will be expected to develop a research interest and supervise research projects undertaken by doctors in training grades. The post holder may also develop a leading role in clinical audit/clinical effectiveness.

(iv) Administration

The post holder will be expected to undertake such administrative duties as are required for his or her patients and the proper functioning of the department.

e) Description of Service

Cardiac services in RHSC and GJNH

Cardiac services have undergone significant change in the last 12 months. This is a result of additional investment to support the service to meet UK safe and sustainable standards. Included in this have seen expansion in cardiology consultant numbers, additional investment targeted to liaison nursing and clinical psychology plus extra resource to data management and administration/clerical. Service is successfully re-engineering itself across a range of areas including clinical governance, research and development, data management, age appropriate care, psycho social support and transition. Organisational development supporting team ethic within and across departments is also a strong feature of the service work plan. A new nationally commissioned Managed Clinical Network for cardiac services has recently been established with the wider cardiac team featuring significantly in its activities. 2015 offers the cardiac team the opportunity to move to a new build at the Southern General Hospital Campus. Operational plans supporting this move are well established.

The Cardiac Catheter Laboratory has ensured that the department has the technology necessary to allow it to continue at the leading edge of interventional catheter treatment. It has been constructed with hybrid procedures between surgeon and cardiologist in mind, utilizing greater space, temperature control, operating theatre conditions and a tilting/rocking table.

The MRI suite contains a new 1.5 Tesla Phillips magnet. Full cardiac image acquisition and analysis software is in place and there is a dedicated post procedural processing room adjacent to the scanner. Images are now archived on the Scottish PACS system which permits direct transfer and review of images to/from other centres.

The department has the facility for full invasive/non-invasive cardiac electrophysiological assessment and ablation treatment for arrhythmias.

Cardiology services run one stop clinics for new patients, providing review by a cardiologist together with ECG and echo at a single visit. This model of service is now extended to a network of peripheral clinics throughout Scotland allowing local care for a large number of children with heart disease. This system has also allowed training of interested physicians at these sites. In conjunction with the well established Scotland wide telemedicine network this provides the basis for a proposed managed care network extending throughout Scotland.

The Cardiology Department has developed an in-house clinical information and audit system. Since its creation in 1993 it has undergone continual development and is now one of the most advanced cardiac clinical information systems in the UK. It is networked throughout the hospital and acts as an on-line medical record allowing instant access to patient details from enabled sites around the hospital. Information is held on over 14,000 patients including investigation results, cardiac surgery procedure records, diagnostic and interventional catheter lab procedures, inpatient stays, clinic visits, drug therapy and patient correspondence. The system has also been installed in the Royal Hospital for Sick Children Edinburgh and in the Golden Jubilee National Hospital based Adult congenital cardiac unit.

Cardiac Surgery

The RHSC unit provides a national paediatric cardiac surgery service for Scotland. The population of Scotland is just over 5 million and this service operates on patients whose ages range from neonates through to 16 years of age. The paediatric cardiac service was centralised on the Yorkhill site in July 2000. Three consultant paediatric cardiac surgeons provide both this service and adult congenital surgery based at the Golden Jubilee National Hospital. All three surgeons’ job plans are fully committed to the adult congenital and paediatric services. A small amount of paediatric thoracic surgery is undertaken.

The combined cardiology/cardiac surgery ward is shared with the cardiac surgeons and a programme of shared patient care is undertaken. There is a dedicated cardiac theatre nursing team who provide a comprehensive out of hours emergency service in conjunction with a dedicated cardiac anaesthetic service provided by a team of six consultant anaesthetists.

Within the paediatric intensive care unit cardiac surgery and catheter intervention have seven funded beds. Clinical care of the post-operative patients in ITU is shared with ten whole time consultant intensivists and a lead intensivist for cardiac services is in place. Monday to Friday, nine to five there is a dedicated cardiac consultant intensivist present on the unit.

The GJNH houses the Scottish adult congenital cardiac service (SACCS). SACCS beds are funded within the cardiac ward and National Services pod on 3West. The SACCS team includes currently 2 adult congenital cardiologists, 3 dedicated congenital cardiac surgeons, trainees and nurse practitioners. A SACCS MDT is run weekly with teleconferencing facilities to RHSC. There are 19 cardiac intensive care beds within the GJNH. A team of 8 cardiac anesthetists and 10 intensivists provide dedicated 24/7 rotas to cover the cardiac services and intensive care. There is a funded ECLS service for adult congenital patients which includes Cardiac ECMO or VAD support.

The GJNH has a large adult cardiothoracic surgical unit providing cardiac and thoracic surgery services for the West of Scotland. Additionally there is an active national cardiac transplantation programme with an established ECKS service. The SPVU is also based at the GJNH. Excellent collaboration exists between the SNAFS and SPVU and the SACCS team

There is a large interventional cardiology service on the GJNH with 4 Cath Labs and an active primary PCI service.
